Job summary

Are you a Mental Health Practitioner who wants to make a difference to adults living with a mental illness in Shropshire?

We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Practitioner to join our Community Mental Health Services within North Shropshire Locality.

This is an exciting opportunity to be part of a newly developing, dynamic and supportive multi-disciplinary team delivering an enhanced offer in partnership with colleagues to older adults who are experiencing an functional mental illness. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.

Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;

  • Provide specialist assessment and care planning in conjunction with wider community services, service users and carers
  • Evaluate treatment plans
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Co-ordinate the work of the Team
  • Take responsibility for managing a caseload.

Main duties of the job

  • Take responsibility for the management of a complex caseload, which includes, specialist assessment, care planning in conjunction with service user and carer, evaluation of individual treatment plans
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administer medication (Nurses only), complying with relevant Trust procedures and NMC Guidance
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and pathway guidance including; pharmacological and psychologically informed interventions.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
