The Team Leader role supports home management in delivering high-quality care, fostering a safe, caring, innovative, open, supportive, and inclusive environment. Responsibilities include leading the care planning process, transitioning residents smoothly, monitoring resident well-being, maintaining care standards, and practicing safe work systems. Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other responsibilitieswillinclude:

• Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
• Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
• Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
• Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidatemust:

• Have anNVQ 3 or equivalent in care for care homes.
• Two years experience working in a senior position with the resident group.
• Have the ability tocommunicate effectively both verbally and in writing.
• Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
