Job summary

The post holder will work as a Salaried General Practitioner within The Limes Medical Centre, providing high-quality, safe and patient-centred medical care to the practice population.

The successful candidate will work collaboratively with the practice team and wider multidisciplinary team to provide effective management of acute and chronic conditions, preventative healthcare and appropriate onward referral.

Main duties of the job

Clinical Duties

  • Undertake face-to-face and telephone consultations as required.
  • Assess, diagnose and manage a wide range of acute and chronic medical conditions.
  • Develop appropriate management and treatment plans in partnership with patients.
  • Prescribe safely and appropriately in accordance with national and local guidelines.
  • Review and act upon laboratory, imaging and other diagnostic results.
  • Make appropriate referrals to secondary care, community services and other healthcare professionals.
  • Provide appropriate follow-up and ongoing management of patients.
  • Undertake home visits where clinically appropriate and in accordance with practice arrangements.
  • Provide preventative healthcare, health promotion and lifestyle advice.
  • Support the management and review of patients with long-term conditions.
  • Recognise and appropriately manage patients presenting with urgent or emergency conditions.
  • Maintain accurate, contemporaneous and comprehensive clinical records using EMIS Web.
  • Ensure appropriate clinical coding and documentation is completed.

Safeguarding

  • Recognise and respond appropriately to safeguarding concerns involving children and vulnerable adults.
  • Follow practice safeguarding policies and procedures.
  • Work collaboratively with the practice safeguarding team and external agencies where appropriate.

Administrative Duties

  • Review and action clinical correspondence, including hospital letters, discharge summaries and investigation results.
  • Complete clinical tasks within agreed practice timescales.
  • Undertake medication reviews and contribute to safe and effective prescribing.
  • Complete appropriate reports, forms and medical documentation.
  • Participate in the management of incoming clinical workflow.

Quality Improvement and Clinical Governance

  • Participate in clinical audits and quality improvement activities.
  • Contribute to the achievement of QOF, Enhanced Services and other relevant practice targets.
  • Participate in significant event reviews and learning from incidents.
  • Support the practice in maintaining high standards of clinical governance and patient safety.
  • Work within GMC professional standards and all relevant NHS guidance and legislation.
  • Contribute to the practice's preparation for and compliance with CQC requirements.

Team Working

  • Work collaboratively with GPs, nurses, pharmacists, healthcare assistants, social prescribers, care coordinators and administrative staff.
  • Provide appropriate clinical advice and support to members of the wider practice team.
  • Attend practice and multidisciplinary team meetings where required.
  • Communicate effectively and respectfully with colleagues, patients and external healthcare professionals.
  • Support medical students, GP trainees and other learners where appropriate and agreed with the practice.

Professional Development

  • Maintain full GMC registration and a licence to practise.
  • Maintain inclusion on the NHS England Medical Performers List.
  • Participate in annual appraisal and revalidation requirements.
  • Maintain professional knowledge and competence through appropriate Continuing Professional Development (CPD).
  • Keep up to date with developments in clinical practice, prescribing guidance and NHS requirements.
