Job summary
We are seeking a motivated and compassionate Care Coordinator to join West Northumberland Primary Care Network (PCN) on a fixed-term 12-month contract, working 28 hours per week.
This is a rewarding, patient-facing role focusing on improving patient experience, supporting individuals to access the right care at the right time, and helping people navigate health and care services.
Working as part of a well-established multidisciplinary team, you will contribute to delivering personalised care and improving outcomes for patients with a range of health and wellbeing needs. You will work closely with GP's, Social Prescribing Link Workers, Health and Wellbeing Coaches, and other healthcare professionals to ensure patients receive coordinated, person-centred support.
The successful candidate will be highly organised, an excellent communicator, and passionate about helping people take a more active role in managing their health and wellbeing while improving access to local services and support.

Main duties of the job
As a Care Coordinator, you will work closely with patients, carers, GP practices, and healthcare professionals to ensure individuals receive coordinated, person-centred support.
Key responsibilities include:
- Coordinating care and support for patients with complex or long-term health needs.
- Supporting patients to access appropriate health, social care, and community services.
- Developing and maintaining personalised care and support plans.
- Managing appointments, referrals, and communication across services.
- Supporting multidisciplinary team (MDT) meetings and care planning activities.
- Contributing to the coordination of cancer care pathways, including supporting patients through screening, referral, and follow-up processes.
- Working collaboratively with Social Prescribing Link Workers, Health and Wellbeing Coaches, and Primary Care colleagues to improve patient outcomes.
- Identifying and supporting patients who may benefit from additional health and wellbeing interventions.
Job description
As a Care Coordinator within West Northumberland Primary Care Network, you will play a key role in supporting patients to access appropriate health, care and community services, ensuring they receive coordinated and personalised support tailored to their individual needs.
Working closely with GP practices and members of the multidisciplinary team, you will proactively identify and support patients who may benefit from care coordination. You will act as a central point of contact, helping patients and carers navigate services, manage appointments, access relevant support, and develop personalised care and support plans.
The role involves building strong relationships with patients, healthcare professionals and partner organisations to promote joined-up care and improve patient outcomes. You will contribute to the coordination of multidisciplinary team meetings, support the management of patient caseloads, and ensure that care plans and relevant information are effectively communicated and recorded.
You will also support the delivery of key Primary Care Network priorities, including cancer care coordination and, where required, work associated with Enhanced Health in Care Homes. This may include supporting patients through screening and referral pathways, assisting with follow-up activity, and helping individuals and their families understand and access the support available to them.

West Northumberland Primary Care Network is a collaborative partnership of 12 GP practices working together to improve the health and wellbeing of approximately 67,000 patients across West Northumberland.
Our PCN brings together a wide range of healthcare professionals, including GPs, pharmacists, paramedics, care coordinators, social prescribing link workers, health and wellbeing coaches, and nursing teams, to deliver high-quality, patient-centred care closer to home.
