Job summary

Diamond Health Group is seeking a motivated and compassionate Health and Wellbeing Coach to join our skilled and supportive multidisciplinary team.

The successful candidate will work closely with patients to understand their individual health and wellbeing needs, providing personalised support and encouragement to help them make positive, sustainable changes to their health and lifestyle.

The role will involve supporting patients to identify their own goals, build confidence and motivation, and take proactive steps towards achieving the objectives outlined in their personalised health and care plans. You will work collaboratively with patients and the wider healthcare team to promote self-management, prevention and improved health outcomes.

This is a 15-hour-per-week position, ideally worked across three 5-hour days. The working pattern is flexible and can be discussed further at interview.

Main duties of the job

Working as part of our Health and Wellbeing Coaching team, you will:

  • Provide one-to-one and group health coaching support by telephone, face to face at the surgery and, where appropriate, through home visits.
  • Manage and prioritise an allocated caseload according to the individual needs, priorities and level of support required by patients.
  • Support patients to identify personalised health and wellbeing goals and take proactive steps towards achieving sustainable lifestyle changes and improving their overall health and wellbeing.
  • Provide timely, compassionate support to patients experiencing mental health concerns, offering a safe listening service, appropriate information and signposting to relevant services and interventions.
  • Gather relevant information, identify any immediate concerns or risks to patient safety, and escalate concerns to an appropriate clinician where required.
  • Support local health, social care and voluntary sector professionals to make appropriate referrals to the Health and Wellbeing Coaching service.
  • Promote and raise awareness of the Health and Wellbeing Coaching service across the practice and wider community.

  • Work sensitively with people, their families and carers to gather key information
  • Coach individuals across a series of sessions to set personal goals and build the skills and confidence to then achieve them
  • Work with the principles of self-management to actively support shared decision making, proactive engagement with social prescribing and support self-management education
  • Promote health coaching and it's role in self-management as part of personalised care
  • Be proactive in developing strong links with local organisations
