Job summary
Diamond Health Group is looking to recruit two dynamic and highly motivated Patient Care Coordinators to join our well-established team.
This is an exciting and varied role within a busy primary care environment, where no two days are the same. The successful candidates will be patient-focused, approachable and committed to providing a high standard of care, support and service to our patients.
Both positions are part-time, 20 hours per week, with flexibility around the working pattern for the successful candidates. Working arrangements will be discussed at interview and confirmed upon appointment.
There will be an expectation to work one late shift each week on a Friday, with a finish time of 6:30pm.

Main duties of the job
You will assist and direct patients in accessing the appropriate service, or healthcare professional in a courteous, efficient and effective way.
You will provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or by telephone.
The roll will provide full administration support for the practice and assist with the processing of prescription requests when required.

Job Summary
Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice.
Deal with all general enquiries, explain procedures.
Signpost new and follow-up appointments by telephone and online consult to manage an efficient appointment service
Process post, scanning and accurate coding of documents.
Process prescription requests in accordance with the current repeat prescribing protocol.
Receive prescription queries from patients, pharmacies and all members of staff, and to understand who to approach for help.
Liaise with pharmacies regarding queries, updating them on medication changes to blister packs, and arranging any urgent action required.
To uphold and deliver the values, standards, aims, objectives, and expectations of the Diamond Health Group.
To act as a focal point of communication between all members of staff, patients, pharmacies, and hospitals, to ensure patients repeat medication is produced within the current protocols of the surgery.
