Job summary

We are seeking a motivated and patient-centred Paramedic to join West Northumberland Primary Care Network. This is an exciting opportunity to work within primary care, using your clinical skills to assess, diagnose and manage a wide range of patient needs while working as part of an experienced multidisciplinary team.

You will play a key role in delivering high-quality, person-centred care, supporting same-day access services, home visiting, urgent clinical assessment and long-term condition management. Working within your professional scope of practice and supported by the GP Paramedic Lead, you will help improve patient outcomes while contributing to the ongoing development of primary care services.

Main duties of the job

As a Primary Care Paramedic, you will be responsible for assessing, diagnosing and managing patients presenting with acute and long-term health conditions. You will provide home visits, support same-day care and undertake clinical assessments and investigations within your scope of practice.

Working closely with GPs, practice teams and wider community services, you will promote self-management, develop clinical management plans, make referrals where appropriate and contribute to multidisciplinary team working. You will also be responsible for maintaining accurate patient records and delivering safe, effective and compassionate patient care.

When employed under the Additional Roles Reimbursement Scheme (ARRS), the post-holder will fulfil the requirements outlined within Annex B (Section B13) of the Network Contract DES Specification 2026/27.

Job description

  • Work as part of a multidisciplinary team within West Northumberland Primary Care Network.
  • Assess, diagnose and manage patients presenting with a range of acute and long-term health conditions.
  • Provide same-day clinical assessment and urgent care support.
  • Undertake home visits and develop appropriate management and treatment plans.
  • Support the delivery of person-centred care and promote patient self-management.
  • Carry out clinical assessments, investigations and health reviews within scope of practice.
  • Refer patients appropriately to primary, community, secondary care and voluntary sector services.
  • Support the delivery of anticipatory care planning and preventative healthcare initiatives.
  • Maintain accurate, timely and contemporaneous clinical records.
  • Work collaboratively with GPs, nurses, pharmacists and wider healthcare professionals.
  • Contribute to multidisciplinary team meetings and service development activities.
  • Ensure care is delivered in line with local and national policies, guidelines and best practice.
  • Participate in clinical supervision, training and continuing professional development.
  • Promote a safe, effective and compassionate approach to patient care.
  • Support quality improvement activities and the ongoing development of PCN services.
