We are seeking an experienced and motivated Practice Nurse to join our clinical team at St James Medical Centre. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population across both of our practices in Croydon and New Addington.
The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community.
This is a varied role requiring clinical expertise, good judgement and the ability to work independently while maintaining effective communication with colleagues and patients. The ideal candidate will be committed to delivering excellent standards of nursing care and supporting the continued development of primary care services.

The Practice Nurse will provide a range of clinical services to patients registered with St James Medical Centre, working as part of the wider healthcare team to deliver safe and effective care.
The post holder will undertake nursing assessments, develop care plans and support the management of patients with long-term conditions, including diabetes, respiratory disease, cardiovascular disease and hypertension. They will deliver preventative healthcare services including immunisations, cervical screening, NHS health checks and health promotion advice.

Job responsibilitiesClinical Responsibilities
- Provide safe, effective, and patient-centred nursing care in a primary care setting
- Undertake long-term condition management clinics including (but not limited to) diabetes, asthma, COPD, hypertension, and cardiovascular disease
- Perform cervical screening in line with the NHS Cervical Screening Programme
- Administer childhood and adult immunisations and vaccinations
- Carry out wound care, dressings, and minor injury management
- Support health promotion and disease prevention activities, including lifestyle advice and patient education
- Identify and escalate safeguarding concerns in line with practice policies
- Maintain accurate, contemporaneous clinical records using EMIS Web
Professional Responsibilities
- Work within the NMC Code of Professional Conduct at all times
- Maintain up-to-date professional registration, revalidation, and CPD
- Participate in clinical supervision, appraisal, and personal development planning
- Actively engage in training, reflective practice, and continuous learning
- Support students, new starters, and colleagues as appropriate
Quality, Governance, and Safety
- Contribute to QOF, enhanced services, and local incentive schemes
- Participate in clinical audit, quality improvement, and service evaluation
- Maintain high standards of infection prevention and control
- Comply with all practice policies, procedures, and clinical governance frameworks
- Support CQC standards, including Safe, Effective, and Well-Led domains
Team Working and Communication
- Work collaboratively with GPs, pharmacists and the wider PCN team
- Communicate effectively with patients, carers, and colleagues
- Contribute positively to a supportive and respectful team culture
- Use digital systems and data to support clinical decision-making and service improvement
Technology and Innovation
- Use clinical systems (EMIS Web, Accurx, NHS systems) effectively and accurately
- Engage with digital tools and new ways of working to improve efficiency and patient experience
- Support data-driven approaches to long-term condition management and recall systems
Confidentiality and Information Governance
- Maintain patient confidentiality at all times in line with GDPR and NHS guidance
- Ensure information is recorded, stored, and shared appropriately
