This post will support the Leeds Neighbourhood Diabetes Pilot, an innovative programme designed to improve access to specialist diabetes care, reduce health inequalities and provide earlier intervention for people living with diabetes.

Working within the Tier 2 Neighbourhood Support Team, you will provide specialist podiatric assessment and management for adults with diabetes and multimorbidity across the Beeston, Middleton & Hunslet PCN footprint.
