As part of a dynamic and supportive multidisciplinary team, you will provide specialist assessment, treatment, care coordination, and ongoing support to people experiencing a range of complex mental health difficulties within the local community.

Working within a recovery-focused model of care, you will hold a caseload and work collaboratively with service users, their families, and carers to promote recovery, build resilience, and support individuals to achieve their personal goals and maximise their independence. You will work alongside psychiatrists, psychologists, occupational therapists, support workers, and third-sector partners to deliver high-quality, person-centred care.

We are seeking an enthusiastic, compassionate, and motivated practitioner who is committed to delivering excellent care and achieving positive outcomes for service users. You will be flexible, resilient, and confident working within a multidisciplinary environment, with a passion for continuous learning, service improvement, and innovation.

At Sussex Partnership NHS Foundation Trust, our vision is to provide outstanding care and treatment for the communities we serve. Our staff are at the heart of making this vision a reality.

Main duties of the job

The Chichester Neighbourhood Mental Health Team is a dynamic and forward-thinking community mental health service dedicated to supporting adults living with severe and enduring mental health difficulties. Working within a recovery-focused model, you will manage a caseload of service users, empowering them to build resilience, develop positive coping strategies, and maximise their independence and quality of life.

You will be part of a skilled and supportive multidisciplinary team, working alongside psychologists, psychiatrists, nurses, occupational therapists, support workers, and valued third-sector partners to deliver holistic, person-centred care.

We are passionate about placing people at the heart of everything we do and actively involve service users, families, and carers in care planning and service development. We welcome applicants who have experience of, or a commitment to, co-production and collaborative working.

As a team, we are committed to continuous improvement, innovation, and delivering high-quality, compassionate care. If you are motivated by making a meaningful difference in people's lives and want to be part of a supportive and progressive service, we would love to hear from you.

This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

Job description

This role is focused on improving outcomes for people living with severe and enduring mental health difficulties. You will work collaboratively with service users, their families, and carers to co-produce personalised care and risk management plans that promote recovery, enhance wellbeing, and support individuals to live as independently as possible.

Success in this role relies on building and maintaining effective partnerships with a wide range of services, including GPs, Crisis Resolution and Home Treatment Teams, housing providers, social care, and voluntary and community sector organisations. You will take a proactive and creative approach to ensuring people receive the right care, at the right time, from the most appropriate service. Where required, you will facilitate access to specialist services, including Early Intervention in Psychosis and Eating Disorder Services.

As a valued member of the multidisciplinary team, you will:

  • Undertake comprehensive assessments and contribute to the development, implementation, and review of recovery-focused care plans.
  • Actively participate in multidisciplinary discussions, formulation, risk management, and care planning.
  • Manage a caseload and work collaboratively with service users, families, and carers to achieve positive outcomes.
  • Contribute to service development, quality improvement initiatives, and the implementation of evidence-based practice.
  • Participate in regular clinical, professional, and managerial supervision.
  • Maintain high standards of professional practice, working in accordance with Trust policies, professional standards, and ethical frameworks.
  • Support individuals to access local health, social care, and community resources that promote recovery and inclusion.
  • Ensure accurate and timely record keeping in line with Trust standards, national guidance, and NICE recommendations.

What we offer

We are committed to supporting your professional development and wellbeing through:

  • Regular management, clinical, and professional supervision.
  • Access to a wide range of training, learning, and continuing professional development opportunities.
  • A supportive and experienced multidisciplinary team environment.
  • Opportunities to contribute to service innovation, quality improvement, and the ongoing transformation of community mental health services.

We are seeking an enthusiastic, compassionate, and motivated practitioner with excellent communication and interpersonal skills, strong clinical knowledge, and a commitment to delivering high-quality care in the community. You will be flexible, resilient, and committed to recovery-focused, person-centred care, with the ability to work effectively as part of a multidisciplinary team.

About us

Over the next five years, community mental health services are going to be undergoing a real transformation and you could be part of these exciting developments. Our community mental health teams are becoming part of a much more integrated network of services and our mental health clinicians will be central in driving the changes, in close partnership with our primary and third sector colleagues.

We recognise that working in mental health can be stressful so we prioritise staff wellbeing, providing regular supervision, reflective practice, mentoring, and any other support that our clinicians need to ensure their wellbeing.

If you are a band 5 or newly qualified nurse with previous mental health experience who has a desire to develop into a community mental health nurse role, then please contact us as we would be delighted to talk with you about any development opportunities.

We encourage flexible working. We know that a positive work/ life balance brings about huge health-related benefits, which has a positive impact on the care we provide. Please speak to us about flexible working, for example, home-working, part time hours, flexible start/ finish times or anything else. If it works for you and works for the role, we'll do our best to make it happen.
