This is a unique opportunity to help shape innovative models of care focused on admission prevention, virtual wards, rapid response and community respiratory management. Working across organisational boundaries, you will support patients with complex respiratory conditions to receive high-quality care closer to home, reducing unnecessary hospital attendance and admissions.

As a senior clinician, you will work collaboratively with primary care, acute and community services to develop new pathways that improve patient outcomes and experience. You will provide expert clinical assessment, leadership and decision-making while contributing to service development and quality improvement.

We are looking for an adaptable, forward-thinking individual with excellent clinical, communication and leadership skills who is passionate about patient-centred care and service transformation.

This role will support the development of the Solihull Respiratory Hub, virtual ward services and innovative models of respiratory care across the locality.

Applicants should have at least two years' Band 6 respiratory experience, advanced clinical assessment skills and expertise in respiratory disease management, including competency in performing and interpreting Capillary Blood Gases (CBGs) and managing oxygen therapy.

Main duties of the job

  • Undertake advanced respiratory assessments to support diagnosis and develop personalised management plans.
  • Initiate and optimise evidence-based respiratory treatments and medications within scope of practice.
  • Perform and interpret Capillary Blood Gas (CBG) analysis to support oxygen therapy and clinical decision-making.
  • Assess and manage patients with acute respiratory symptoms in community, clinic and virtual ward settings.
  • Support admission avoidance pathways through timely assessment, intervention and care coordination.
  • Determine appropriate care pathways, including safe management at home where suitable.
  • Work autonomously and within a multidisciplinary team to deliver high-quality, patient-centred care.
  • Provide specialist input to the Solihull Virtual Ward, helping prevent unnecessary hospital admissions and facilitate early intervention.
  • Provide education and self-management support to patients, carers and healthcare professionals.
  • Act as a specialist respiratory resource for conditions including COPD, bronchiectasis, asthma and ILD.
  • Deliver advice on inhaler technique, airway clearance, oxygen therapy, symptom monitoring, smoking cessation and lifestyle changes.
  • Contribute to service development, quality improvement and the implementation of innovative respiratory pathways across Solihull.
  • Develop individualised care plans that promote independence, improve outcomes and reduce avoidable hospital admissions
