Health& Social Care Assessor

Base: County Hospital

Hours: 22.5 hours

An exciting opportunity has arisen for an enthusiastic and self-motivated individual to join the Integrated Discharge Hub at County Hospital to support Winter extension plans in the assessment support we deliver.

The role of the Health& Social Care Assessor will be based in our Triage hub at County Hospital, providing discharge guidance and support to our referring acute hospital trust.

The successful candidate will be part of a triage team that receive referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients' needs as part of a multi-disciplinary team with a key role in decision making.

Main duties of the job

The post holder will liaise with a variety of people (service users, colleagues, member of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual's ability/potential for reablement via the discharge to assess pathways alongside supporting admission avoidance assessments to prevent a hospital admission.

The post holder will be working autonomously as part of a hub and will be expected to be able to use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams.

The service currently operates a 6 day a week service through the hours of 9am to 6pm including Saturdays and Bank Holidays. This would be negotiated on a pro rata basis.

We are looking for someone that has a passion for making a difference to our patients, someone who wants to enable individuals to live their best lives using our reablement, recovery and rehabilitation service. The successful candidate will be an enthusiastic and innovative individual who possesses effective telephone and communication skills, IT skills, and exceptional organisational skills.

Job description

The service will be delivered in accordance with the principles of Recovery; empowering individuals to take control of their lives and putting them at the centre of everything we do, recognising the rights of all individuals to gain access to valued roles, relationships, activities and facilities within their local community.

The post holder will facilitate the patients safe transfer from hospital back into the community and to liaise between the hospital and other statutory and non-statutory agencies as required.

The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of Health & Social Care understanding and professional judgement.

The post holder is responsible for working independently, exercising assessment and evaluation skills to full ability in order to adjust care assessments as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patients best interests.
