Join Southampton’s New Review and Reassessment Team

Southampton Adult Social Care is at an important point in its improvement journey. We are strengthening our operational practice, reducing delays and creating a more responsive and sustainable service for Southampton residents.

As part of this work, we are establishing a new Review and Reassessment Team. The team will ensure that people receive timely, proportionate and meaningful reviews of their care and support, with a clear focus on what matters to them, what has changed and how we can help them maintain or regain independence.

This is about much more than completing reviews. It is an opportunity to help shape a new team, strengthen the quality and consistency of social work practice and demonstrate the difference that purposeful reviews and reassessments can make to people, carers and the wider Adult Social Care system.

The opportunity

The team will work with people across Adult Social Care to:

undertake timely, strengths-based reviews and reassessments;

ensure care and support plans remain appropriate and outcome-focused;

identify changes in need, circumstances or risk and respond promptly;

promote independence, choice and connection to family and community;

apply the Care Act, Mental Capacity Act and safeguarding responsibilities confidently;

work collaboratively with carers, providers and partner organisations;

make proportionate, evidence-based and financially responsible decisions; and

contribute learning and insight to the wider improvement of Adult Social Care.

The team will support Southampton’s commitment to providing a high-quality service that is easy to navigate, developing a confident and competent workforce and ensuring services are fair, sustainable and flexible.

About the role

As a Social Worker, you will have purposeful conversations with people and carers about what is working, what has changed and what matters most to them.

You will use your professional judgement to complete proportionate reviews and reassessments, co-produce outcome-focused plans, manage risk and help people draw on their strengths, relationships and community connections. This is a varied role offering opportunities to develop practice in complex decision-making, safeguarding, mental capacity, strengths-based approaches and service improvement.

What we can offer you:

Salary: The salary band for this role is £43,150 - £47,664. The starting salary is £43,150 with annual progression through the salary bands.
