Knowledge of community services and an understanding of community urgent response pathways would be advantageous.

Within the team we have Occupational Therapists, physiotherapists, nurses and medics. We work as part of a collaborative MDT to provide rapid assessment and intervention to people across the Dudley borough.

The successful candidate will be able to work autonomously in the community but will have support from senior staff.
