Job summary
The Advanced Paramedic Practitioner is an experienced autonomous clinician who provides proactive, responsive and coordinated healthcare for housebound residents across Marlow Medical Group.
The post-holder will work collaboratively with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Nursing Teams, Housebound Staff, Community Services and Secondary Care Providers to deliver high-quality, person-centred care to some of the most vulnerable and medically complex patients in the healthcare system. The role will support delivery of the Primary Care Network (PCN).
Main duties of the job
The role combines advanced clinical assessment, urgent care, long-term condition management, proactive care planning, frailty management, admission avoidance, end-of-life care support, and multidisciplinary care coordination.
The post-holder will provide a consistent clinical presence within the local community, helping reduce avoidable hospital admissions, improve continuity of care, support medicines optimisation, and improve outcomes for residents through proactive intervention and integrated working.

Key Responsibilities
1. Clinical Responsibilities & Advanced Practice
Provide autonomous assessment, diagnosis, treatment and management of housebound residents presenting with acute, chronic and complex healthcare needs.
The post-holder will:
Undertake comprehensive holistic clinical assessments.
Perform clinical examinations and advanced assessment.
Formulate differential diagnoses and management plans.
Manage acute illness and clinical deterioration.
Deliver urgent and same-day care assessments.
Assess and manage frailty syndromes.
Assess residents following falls.
Support management of long-term conditions.
Undertake home and housebound ward-round consultations.
Support end-of-life and palliative care patients.
Identify and escalate safeguarding concerns.
Request and interpret appropriate investigations.
Manage residents with multimorbidity and complex care needs.
Undertake anticipatory care planning.
Recognise and manage clinical risk and deterioration.
Coordinate onward referral where required.
Provide telephone and digital clinical consultations.
Work within professional scope of practice and competence.
2. Equity of Access to care for Housebound
The post-holder will:
Participate in regular multidisciplinary housebound ward rounds.
Support identification of residents requiring proactive review.
Maintain and review personalised care and support plans.
Support development and review of anticipatory care plans.
Identify residents at high risk of admission or deterioration.
Coordinate intervention plans for vulnerable residents.
Support implementation of NHS Comprehensive Geriatric Assessment principles.
Undertake routine and responsive housebound reviews.
Reduce avoidable emergency admissions and A&E attendances.
Support delivery of structured medication review processes alongside Clinical Pharmacists.
Promote optimal hydration, nutrition and falls prevention initiatives.
Support implementation of enhanced care planning processes.
Contribute to improving end-of-life care outcomes.
Participate in Gold Standards Framework discussions where appropriate.
Support delivery of population health management approaches for patients who are housebound.
3. Urgent Care & Admission Avoidance
Provide a rapid response clinical service to housebound residents. The post-holder will:
Assess deterioration in housebound residents.
Undertake urgent same-day assessments.
Manage acute presentations where appropriate.
Assess residents following falls.
Support clinical decision-making regarding hospital admission.
Implement admission avoidance strategies.
Liaise with ambulance services, community teams and secondary care providers.
Monitor residents following acute episodes.
Support safe discharge and post-discharge review.
Reduce unnecessary hospital attendance through proactive care delivery.
4. Frailty, Long-Term Conditions & Population Health
Provide expert management of residents living with frailty and long-term conditions. The post-holder will:
Support delivery of frailty pathways.
Undertake comprehensive geriatric assessments.
Manage patients with:
Frailty
Dementia
Diabetes
Cardiovascular disease
Heart failure
COPD
Asthma
Hypertension
Chronic kidney disease
Mental health conditions
Complex multimorbidity
Develop and review personalised care plans.
Support preventative healthcare interventions.
Promote healthy ageing initiatives.
Identify residents requiring proactive intervention.
Support reduction in health inequalities.
5. Leadership & Housebound Service Development
The Advanced Paramedic Practitioner will provide clinical leadership across housebound services and contribute to strategic development of proactive care models
The post-holder will:
Act as a visible clinical leader for housebound patients
Support development of housebound clinical pathways.
Promote quality improvement initiatives.
Champion patient safety and learning culture.
Support housebound workforce education.
Contribute to service redesign initiatives.
Support workload and demand management.
Contribute to implementation of PCN and organisational objectives.
Lead improvement projects related to frailty and housebound care.
Participate in service evaluation and development.
6. Interprofessional Working & Collaborative Practice
The post-holder will work across organisational and professional boundaries to improve outcomes for residents. The post-holder will:

7. Education, Training & Workforce Development
Support the delivery of high-quality, evidence-based care.
Support education of colleagues
Deliver clinical teaching and training sessions.
Support mentorship of paramedic students.
Participate in clinical supervision.
Share learning from incidents, audits and quality activities.
Support development of multidisciplinary team capability.
Promote evidence-based practice.
8. Digital & Operational Support
The post-holder will:
Maintain accurate and contemporaneous clinical records.
Ensure appropriate use of SNOMED coding.
Utilise EMIS and associated clinical systems effectively.
Manage clinical correspondence and test results.
Support call and recall systems.
Participate in digital-first models of care where appropriate.
Maintain information governance compliance.
Contribute to operational efficiency and service improvement.
9. Research, Audit & Quality Improvement
The post-holder will support continuous improvement of housebound services. The post-holder will:
Participate in clinical audits.
Lead quality improvement projects.
Undertake data collection and service evaluation.
Analyse housebound activity and outcome data.
Participate in significant event reviews.
Support implementation of national guidance.
Contribute to evidence-based service development.
Evaluate outcomes and implement improvements.
