About the Role

The Adults Brokerage Team is seeking a motivated and organised Brokerage Assistant to join our dynamic Team. This is an exciting opportunity to support a service that plays a critical role in ensuring safe, timely, and cost-effective placements for residents.

As a Brokerage Assistant, you will provide essential support to the Brokerage function, assisting in the coordination and administration of placements across a wide range of services including residential and nursing care, homecare, supported living, day opportunities, outreach, and respite.

You will play a key role in maintaining accurate records, supporting payment processes, and ensuring effective communication between social work teams, providers, and internal services.

Key Responsibilities

Support the day-to-day brokerage function, assisting with placement requests, referrals, and coordination of services.

Maintain accurate records on LAS and ContrOCC systems, ensuring information is up to date, compliant, and supports payment processes.

Liaise with providers to confirm service availability, capacity, and placement details, supporting the brokerage process.

Assist with hospital discharge arrangements, including suspensions, restarts, and urgent care coordination.

Support the processing of placement documentation, referrals, and service changes, including increases and decreases in care provision

Work collaboratively with social work teams, internal departments, and external providers, ensuring effective communication and timely service delivery.

Assist in monitoring placement activity and support the identification and resolution of payment and service issues.

Maintain and update placement trackers and data systems, ensuring high-quality management information is available.

Support the team in identifying cost savings and value for money opportunities within placement activity.

Provide flexible support across the team, including covering for other Brokerage Assistants and supporting Brokerage Duty when required.

You will be:

Highly organised, motivated, and able to manage a busy workload in a fast-paced environment

A clear and confident communicator with strong interpersonal skills

Detail-oriented, with a strong focus on accuracy and data quality

Comfortable using IT systems and managing administrative processes

Able to work both independently and as part of a team

You may also have:

Experience of working within social care, administration, or customer-focused environments

Knowledge or experience of LAS and ContrOCC systems (desirable)
