Job summary

Glenpark & Teams Medical Services has an exciting opportunity for an enthusiastic, experienced Practice Nurse to join our friendly and committed nursing team. The position is for up to 37.5 hpw - full time or part time considered - working across both of our sites in Dunston and Teams.

You will be joining a great team lead by an experienced ANP, working alongside a Nurse Practitioner, 2 Practice Nurses, 2 Nurse Associates, a Healthcare Assistant and 2 Phlebotomists. Regular mentoring sessions will be provided, along with support for both career and personal development within the role/team.

The Practice Nurse will work delivering person-centred long term condition management to our patients, and depending on experience and skills could also see patients with acute respiratory/minor illness following triage by the on-call team. You will work both independently and as part of a multi-disciplinary team, using expert clinical judgement and decision making to ensure safe and effective care.

We would welcome visits to discuss the role further and for you to meet the team. We would also be interested to hear of any skills you would wish to develop or bring to the team.

Main duties of the job

The Practice Nurse will work autonomously and as part of the wider team to deliver high-quality care, including:

  • Management of long-term conditions, including diabetes, asthma, COPD and Ischaemic Heart Disease
  • Providing follow-up and continuity of care for patients
  • Delivering health promotion, preventative care, and self-management support
  • Supporting the practice in achieving QOF targets and enhanced services
  • Contributing to service development and quality improvement initiatives

The post holder will also support the practices commitment to Year of Care and personalised care planning, particularly for patients with long-term conditions.

Main duties of the job

  • Support the management of long-term conditions, including diabetes, asthma, COPD and ischaemic heart disease working closely with the nursing team
  • Provide appropriate health promotion for patients with or at risk of a long- term conditions
  • Manage & treat long-term conditions in line with the most current national & local guidelines
  • Cervical smear taking
  • Providing all immunisations, including:
  • Routine childhood vaccinations
  • Adult vaccinations
  • Seasonal vaccinations (e.g. flu, COVID-19)
  • Travel vaccinations and travel health advice
  • Supporting the practice in achieving QOF targets and enhanced services
  • Contribute to the provision of education & training for other staff & students
  • Participate in research & audit where appropriate
  • Maintaining accurate and contemporaneous records at all times
  • Attend and participate in any meetings when required
  • Any other delegated duties appropriate to the role
  • Flexibility of working to ensure sufficient cover is provided for nurse holidays/sickness
  • Maintain effective liaison with other agencies & staff concerned with patient care

We are a GMS partnership with two practice sites, serving a list of approx. 15,000 patients.

Glenpark Medical Practice and Teams Medical Practice are both well established, high-achieving practices and are part of Gateshead Inner West PCN and consistently maintain a high QOF achievement.

The practice comprises 6 GP Partners and 7 salaried GPs, supported by the Practice Managers/Assistant Practice Manager. We have an established nursing team, and a highly skilled and responsive reception and administrative team.

As a practice, we support the nursing team throughout their professional development as well as also being a training practice for Doctors in Training and medical students. The practice holds a strong belief in the importance of continuous professional development as well as personal wellbeing.

The practice also specialises in substance misuse, provides a minor surgery service and LARC service.

We work closely with our PCN team comprising Pharmacists, Pharmacy Technicians, Physiotherapists, Mental Health Nurse and Social Prescribing Link Workers.

We have close working relationships with our community colleagues and have an active Patient Participation Group.
