Main duties of the job

The Advanced Nurse Practitioner will work as an autonomous, highly skilled clinician within Collingwood Health Group, providing advanced clinical assessment, diagnosis, and management of patients presenting with acute and undifferentiated conditions. The post holder will deliver high-quality, patient-centred care through same-day and urgent appointments, including the management of minor illness, minor injury, and acute exacerbations of chronic disease.

Working within their scope of practice and professional regulatory framework, the Nurse Practitioner will independently prescribe, request and interpret investigations, initiate treatment plans, and ensure appropriate follow-up or escalation of care where required.

The role requires strong clinical decision-making, effective risk management, and the ability to work both autonomously and collaboratively within a multidisciplinary primary care team. The post holder will contribute to service development, clinical governance, and continuous quality improvement, while maintaining accurate clinical records and upholding the highest standards of professional practice, confidentiality, and patient safety.

The Nurse Practitioner will support health promotion, patient education, and preventative care initiatives, contributing to improved patient outcomes and enhanced access to timely primary care services.

You will work with the team to develop new and innovative concepts, models, methods and practices to deliver Primary Care.

Job description

Responsibilities of the Advanced Nurse Practitioner will include the following:

• Work independently but as part of a multidisciplinary clinical team.
• Provide care of patients with a range of both chronic and acute medical needs, providing subject matter expert advice.
• Be an Independent Prescriber and support patients in the use of their prescribed or over the counter medicines.
• Assess patients with undiagnosed, undifferentiated medical conditions and make treatment decisions, including ordering necessary investigations.
• Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence.
• Provide both face to face and telephone consults.
• Maintain accurate clinical entries within the electronic patient records, ensuring coding is effective and in line with QOF and practice policy using templates eg Ardens as appropriate.
• Request, collect, process and interpret pathology and other test results as required.
• Review, process and action documentation relating to patients where necessary.
• Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient’s health.
• Support the team in dealing with clinical emergencies.
• Recognise, assess and refer patients presenting with mental health needs.
• Take part in vaccination programmes for adults and children as required.
• Contribute to practice targets (QOF etc.), complying with local and regional guidance.
• Liaise with external services/agencies to ensure the patient is supported appropriately.
• Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
• Contribute to implementation of health promotion and wellbeing programmes.
• Support the clinical team with all safeguarding matters in accordance with local and national policies.
• Understand practice and local policies for substance abuse and addictive behaviour, referring or signposting patients appropriately.
• Deliver opportunistic health promotion where appropriate.
• Participate in local initiatives to enhance service delivery and patient care.
• Support and participate in shared learning within the organisation.
• Consider developing an area of specialist interest, taking the lead within the organisation if appropriate.
• Continually review clinical practices, responding to national policies and initiatives where appropriate.
• Participate in the review of significant/ learning events.
• Contribute to development of practice administrative and clinical protocols in line with the needs of the patient and current legislation.

Our multidisciplinary team is led by 9 GP partners, and supported by a wide ranging multidisciplinary team that includes, Salaried GPs, Advanced Nurse Practitioners, Advanced Care Practitioners, Physician Associate, Practices Nurses, Nurse Associate and Healthcare Assistants.

We are also fortunate to have an integrated pharmacy team comprising of clinical pharmacists and pharmacy technicians who contribute significantly to the safe and effective management of prescribing and medicine use.
