Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.

You have current registration with Social Work England

You have completed your AYSE

You have an in-depth knowledge of current legislation and guidance, as well as current national policy and practice developments

You have a full Drivers Licence and the ability to travel between different locations

You are willing to learn new things and able to maintain your own continuing professional development.
