Job summary

A fantastic opportunity has arisen for an experienced Practice Nurse (Band 6) to join Oakley Health Group, a large and progressive GP practice serving over 29,000 patients across Yateley and Blackwater.

We are looking for an enthusiastic, motivated and experienced Practice Nurse who is passionate about delivering high-quality, patient-centred care and enjoys working as part of a supportive multidisciplinary team.

This is a varied clinical role offering the opportunity to use and develop a broad range of Practice Nursing skills, including treatment-room nursing, immunisations, cervical screening, wound care, clinical assessment and long-term condition management.

We can offer full-time or part-time hours, up to 37.5 hours per week. The successful applicant will need to be available to work one evening per week until 8.00 pm, with Saturday working on a rota basis.

Main duties of the job

The successful applicant will:

• Provide safe, effective and evidence-based nursing care to our patient population.
• Undertake a range of core Practice Nursing activities, including adult and childhood immunisations, cervical screening, wound care, dressings and routine treatment-room procedures.
• Undertake routine clinical observations and basic clinical assessment.
• Recognise and appropriately escalate patients who are deteriorating or require further clinical assessment.
• Support the management of patients with long-term conditions according to competence and experience.
• Provide health promotion, screening and preventative healthcare.
• Work autonomously within professional scope of practice and in accordance with NMC standards.
• Work safely and appropriately under relevant PGDs and PSDs.
• Ensure safe medicines management and adherence to practice policies and procedures.
• Maintain accurate, contemporaneous clinical records and use clinical IT systems effectively.
• Follow infection prevention and control procedures and maintain safe cold-chain processes.
• Prioritise and manage a varied clinical workload effectively.
• Work collaboratively with GPs, Advanced Practitioners, Practice Nurses, Healthcare Assistants, Pharmacists and the wider multidisciplinary team.
• Contribute to clinical governance, audit, quality improvement and continuing professional development.
• Support students, trainees and developing members of the nursing team where appropriate.

Job PurposeThe Practice Nurse will work as an autonomous and accountable member of the nursing team, providing high-quality, evidence-based nursing care to the patients of Oakley Health Group.

The post holder will undertake a broad range of Practice Nursing duties, including treatment-room procedures, immunisations, cervical screening, wound care, clinical observations, health promotion, screening and long-term condition management, in accordance with their level of competence and professional scope of practice.

The post holder will work collaboratively with GPs, Advanced Practitioners, Practice Nurses, Healthcare Assistants, Pharmacists, administrative staff and other members of the multidisciplinary team to ensure patients receive safe, effective and coordinated care.

The role requires a flexible and proactive approach, with the ability to work independently, prioritise competing clinical demands and recognise when further assessment, advice or escalation is required.

Key ResponsibilitiesClinical Care

• Provide safe, effective and compassionate nursing care to patients attending the practice.
• Undertake comprehensive clinical observations and basic clinical assessments within professional competence.
• Recognise changes in a patient's condition and take appropriate action, including escalation to a GP, Advanced Practitioner or other appropriate clinician.
• Undertake routine treatment-room procedures, including wound assessment and management, dressings and other procedures appropriate to the role.
• Deliver adult and childhood immunisations in accordance with current guidance, practice protocols and relevant PGDs/PSDs.
• Undertake cervical screening/cytology within current competence and training.
• Provide health promotion, screening and preventative healthcare.
• Support the management and monitoring of patients with long-term conditions, according to competence and experience.
• Provide appropriate patient education, self-management advice and health promotion.
• Maintain patient confidentiality and comply with information governance requirements.

Medicines Management and Clinical Safety

• Administer and supply medicines safely within professional scope of practice and in accordance with relevant legislation, policies, PGDs and PSDs.
• Ensure appropriate assessment of vaccination contraindications and precautions.
• Maintain and monitor vaccine cold-chain procedures in accordance with practice policy and national guidance.
• Follow infection prevention and control procedures at all times.
• Ensure clinical equipment is used safely, appropriately maintained and fit for purpose.
• Report and appropriately manage clinical incidents, safeguarding concerns and risks in accordance with practice procedures.

Long-Term Condition ManagementWhere appropriately trained and competent, contribute to the assessment, monitoring and management of patients with long-term conditions, including:

• Asthma and COPD
• Diabetes
• Cardiovascular disease and hypertension
• Other chronic disease areas according to practice requirements and individual competence

The post holder may also contribute to specialist areas including respiratory assessment, spirometry, women's health, travel health, contraception and sexual health, lower-limb assessment, leg ulcer management, ABPI/Doppler assessment and compression therapy, subject to appropriate training and competence.

Documentation and Information Technology

• Maintain accurate, contemporaneous and clinically appropriate patient records.
• Record all consultations, assessments, interventions and clinical outcomes in accordance with professional and organisational requirements.
• Use the practice's clinical and administrative IT systems effectively.
• Maintain confidentiality and comply with information governance and data protection requirements.
• Ensure relevant clinical information is communicated appropriately to other members of the healthcare team.

Team Working and Communication

• Work effectively as part of the wider multidisciplinary team.
• Communicate clearly and professionally with patients, relatives, carers and colleagues.
• Develop and maintain effective working relationships across the practice.
• Contribute to clinical meetings, team discussions and service development activities as appropriate.
• Support colleagues and contribute to a positive and professional working environment.
• Where appropriately trained, support students, trainees and developing members of the nursing team.

Clinical Governance and Professional Responsibilities

• Work within the standards and requirements of the Nursing and Midwifery Council (NMC).
• Maintain professional registration and practise within the limits of professional competence and scope of practice.
• Maintain and develop clinical knowledge and skills through continuing professional development.
• Participate in clinical audit, quality improvement and service development initiatives.
• Adhere to Oakley Health Group policies, procedures and clinical protocols.
• Maintain awareness of current national guidance and evidence-based practice relevant to primary care nursing.
• Participate in mandatory training and other training identified as necessary for the role.
• Contribute to the development and review of nursing services within the practice.

Scope of the RoleThe Practice Nurse will work autonomously within their professional scope of practice and level of competence. They will be responsible for recognising the limits of their knowledge and skills and seeking appropriate advice or escalating care when required.

The post holder will be expected to maintain high standards of clinical practice and patient safety and to contribute positively to the continued development of nursing services at Oakley Health Group.
