<p>Are you an experienced Occupational Therapist looking for an opportunity to help shape a brand new service and make a lasting difference to people across Medway? HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service from 1 October. As we launch this exciting new service, we are looking for a Specialist Occupational Therapist to join our growing team and play a key role in supporting people to recover, regain independence and remain in the place they call home. This is an exciting opportunity to join a service at the very beginning of its journey. Working alongside health and social care colleagues, you will help us develop and deliver innovative, person-centred rehabilitation and reablement pathways that improve outcomes for local residents and support our Home First approach.</p><h4><br></h4><h4>Package Description</h4><p>You will be joining a passionate and supportive multidisciplinary team that is focused on delivering outstanding care across the Medway community. As part of a newly mobilised service, you will have the opportunity to contribute ideas, influence service development and help shape the future of rehabilitation and reablement provision across the area.
