Job summary

The community respiratory service consists of the respiratory virtual ward, long term respiratory management, home oxygen and pulmonary rehabilitation. The team provides urgent care to people in their homes.

Within the respiratory virtual ward, the team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors.

Following assessments and intervention, if patients require further support to help keep them at home, patients will be admitted to the virtual ward. The virtual ward provides a safe alternative to hospital for patients living with long term respiratory condition through community-based acute health and care delivery

The home oxygen team is a community assessment service for predominantly respiratory adult patients who require assessment and ongoing management of their oxygen requirements in line with BTS guidelines and key standards of practise.

Pulmonary rehabilitation is a prescribed and progressive exercise and education programme designed for people with long term respiratory conditions who experience symptoms of breathlessness and helps them better understand their condition and symptoms to support effective self-management.

The team acts as an expert resource for other healthcare professionals dealing with complex respiratory patients.

Main duties of the job

A key aspect of this role is to support the Clinical Lead with Leadership and Management tasks, Clinical Expertise and Caseload as well as Quality and Governance.

As a team lead you will ensure each team member has access to a clinical supervisor and arrange time for staff to undertake Clinical Supervision, within available resources.

Delegate appropriate tasks and roles to other members of the nursing team, ensuring they have the skills and knowledge to undertake them.

Participate in recruitment and selection of team members.

Actively participate in clinical audit and service improvement activity to improve patient care and patient outcomes

Set and maintain a high personal and professional standard of practice.

Work in close co-operation with the colleagues within the multi-disciplinary healthcare team to improve the care of patients in providing advice, education and support to patients, their families and carers.

Using the Virtual ward referral criteria, accept appropriate patient referrals and redirect inappropriate referrals.

Assess patient needs, plan, implement and evaluate according to individualised patient needs.

Handle and administer medicines according to professional and organisational guidelines to ensure safety and efficacy.

Identify situations of clinical risk and takes appropriate action to ensure a safe environment for patients/clients/families and staff

Encourage patients and their families to maximise their independence and well-being.
