An exciting opportunity has arisen for a Performance Support Assistant in the friendly and supportive admin team, within the Transfer of Care Hub.

We are looking for an individual who is an excellent communicator, is organised and confident using a variety of IT systems. The post holder will need a to Support the team with patient flow, supporting with the outcome of pathway 2 assessments, completing necessary computer systems to support the flow of patients through the discharge process in a timely manner and support with telephone enquiries.

Hours per week: 37.5 (full and part time hours available)

Main duties of the job

The successful candidate will provide essential support to a group of nurses, therapists, social workers and other professionals by communicating with a variety of services, both internally and outside agencies. They will be responsible for collecting, collating and reporting a variety of data to support the organisation.
