Job summary
An exciting opportunity has arisen for a Band 4 Administrative patient care co-ordinator to join Nottingham CityCares Virtual Wards service on a permanent basis working 37.5 hours per week.
We are looking for an experienced, enthusiastic, and motivated administrator to join our Virtual Wards service. A social enterprise organisation such as CityCare offers opportunities for innovative working, and you will be involved in the development of the service.
The post holder will provide a wide range of administrative support mechanisms to the team and will play a pivotal role in the overall success of the team.
Main duties of the job
You must be willing to participate in the management of change and be able to prioritise tasks and manage time effectively. You must also have the ability to manage your own workload with the support of your team leader and work autonomously. A thorough training package will be offered including internal courses, dependant on previous experience and you will have access to support from senior colleagues.
The hours of work will between the hours of 8am and 6pm Monday to Friday.

Job description
Job purpose
- To be an integral part of the delivery of Multi-Disciplinary Team (MDT) meetings and MDT working. Their role includes the identification of patients that may require a health or social care intervention or in complex cases an MDT discussion.
- As part of the Integrated Care system (ICS), City Care is seeking to recruit Care Navigators who will play a crucial role in establishing integrated health and social care across the City.
Dimensions
- The post holder will work within Nottingham Citys Primary Networks (PCNs ) and within the specialist services acting as the named point of information and a guide to processes for health and social care professionals The post holder will be accessible to members of the PCN via a dedicated telephone line between the hours of 8am and 6pm
- The post holder will provide co-ordination, administrative and data management support to members of multi-disciplinary team within the PCNs/services to improve joint working practices leading to more effective patient care.
- The post holder will work collaboratively with Nottingham Health & Care Point.
Key Responsibilities
- To receive referral information from members of the PCNs neighbourhood (Multi-Disciplinary) team, coordinating the appropriate response based on clinical / presenting need within the agreed pathways to the most appropriate service/s
- To be responsible for the co-ordination and to liaise with all relevant statutory and voluntary sector services including the local authority responsible for the patients care to arrange the necessary support.
- To plan and organise straightforward activities whether planned or adjusted, such as staff rotas, events, meetings.
- To assist patients/clients with appointment/clinic times and incidental contact.
- To manage team/service equipment monitoring and servicing, maintaining accurate and up to date records.
- Be responsible for petty cash, office equipment, placing orders and raising purchase orders for the service.
- To be involved in data reporting via robust structured systems both clinical and non-clinical.
- Undertake surveys and audits relative to the area of work.
- Work independently, unsupervised and be able to guide and advise new starters.
- To record citizen interventions on relevant electronic database systems (e.g. SystmOne or other databases if needed. ) and contribute to report generation, analysis and production
General
- To be customer focused when representing the service and ensuring that the reception people are given is supportive, welcoming and helpful.
- To work within the relevant legal frameworks and have an understanding of the Data Protection Act and how this is related to the management of confidential information in accordance with health and social care policy.
- To plan / organize work using own initiative, whilst being able to work as a valuable member of a team.
- Assist in the orientation and induction of new starters for the service explaining the Care Navigator role.
- To have excellent IT skills, to include Microsoft Office, Outlook, and Excel
- To undertake general office duties to support the role.
- To be involved in the marketing of the Integrated Care System within the wider health and social care community, including voluntary and third sector providers
- To participate in individual appraisal and supervision, contributing to the identification of training opportunities
- To work effectively as part of a team to provide cover for other PCNs when required and to be flexible regarding working hours to meet the needs of the service.
- To undertake supervisory responsibilities including supervision and Professional Development Reviews
- To undertake any additional duties as appropriate and delegated by the Access & Navigation Manager or team manager.
- To make judgments regarding referrals, using triage protocols to ensure that citizens are seen by the most appropriate team / service at the right time to meet their needs. Whilst recognizing the need to involve, or seek advice from, more experienced colleagues as necessary for clinical decision making.
