Job summary

An exciting opportunity has arisen to be part of Sheffield's community based Integrated Care Team (ICTT) in the North of the city.

This post is fixed term for 9 months and is part time 22 hours.

This is a city-wide service, operating 8 am - 6 pm, 7 days a week (including Bank Holidays). The successful candidate will be required to work flexibly to achieve this. The service works to facilitate early discharge of patients from hospital and supports patients at home to prevent unnecessary hospital admission, together with providing community-based therapy for patients with a variety of conditions including neurological, orthopaedic, frailty, musculoskeletal and anxiety issues.

This post will be based within a well-established, multi-disciplinary team consisting of: Physiotherapists, Occupational Therapists, Specialist/Senior Therapy Assistants and Administrative staff, with access to mental health nurses and co-located with the Integrated Care Team Nursing.

Main duties of the job

The successful candidate will carry out Physiotherapy and generic therapy assessments, and plan and deliver treatment programmes for patients in their own homes including residential care settings. This involves working closely with therapy assistants; contributing to their training and development and facilitating them to provide care and rehabilitation to a designated caseload. The role also involves ordering, transporting, providing and fitting of equipment and onward referral to other health and social care services.

A generic training programme is provided to ensure competency in basic nursing and Occupational therapy tasks.

Experience in the rehabilitation of older people, access to your own transport and HCPC registration is essential. Experience in community-based services is desirable.

This vacancy is advertised on a fixed term contract basis for the maximum period of 9 months. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
