Practice Nurse
Preferably Diabetes Specialist
We are looking for an experienced and enthusiastic Practice Nurse with a special interest in diabetes to join our friendly primary care team.
The successful candidate will provide high-quality general practice nursing care, with a particular focus on supporting patients with diabetes and other long-term conditions. Practice Nurses with no diabetes training will also be considered.
Key Responsibilities
- Manage long-term conditions according to experience and competence.
- Provide routine practice nursing services, which may include vaccinations, cervical screening, wound care and health promotion.
- Maintain accurate and appropriately coded clinical records.
- Work closely with GPs, pharmacists and the wider multidisciplinary team.
- Participate in clinical audit, quality improvement and development of diabetes services.
- Undertake diabetes reviews, including monitoring HbA1c, blood pressure, renal function, lipids and cardiovascular risk.
- Provide education, lifestyle advice and support for patients with diabetes and pre-diabetes.
- Review and optimise diabetes management within your level of competence.
- Initiate/titrate diabetes treatments, including insulin where appropriately qualified and competent.
- Support achievement of QOF and other diabetes quality standards.
Person Specification
Essential:
- Registered Nurse with current NMC registration.
- Experience working as a Practice Nurse or in a relevant primary/community care setting.
- Excellent communication, organisational and patient-care skills.
- Ability to work independently and as part of a multidisciplinary team.
Desirable:
- Good knowledge and experience of diabetes management.
- Recognised diabetes qualification or additional diabetes training.
- Experience as a Practice Nurse.
- Competence in insulin initiation/titration.
- Experience with SystmOne.
- Cervical screening and immunisation competencies.
