Job summary

The successful candidate will be enaged with delivery of the nursing element of general medical services at Golborne Canalside Health Centre; long-term condition review, immunisation, cervical screening, treatment room care, health checks and health promotion. The post-holder works within their own competence and NMC scope of practice, supports the practice's QOF and enhanced service commitments, and contributes to a service that delivers excellent healthcare to our patients and to the wider community.

The days/hours the successful candidate would be working are:

Mondays: 0900 - 1300

Wednesday: 1400 - 1800

Thursday: 0900 - 1300

Main duties of the job

• Diabetes: annual & interim review, HbA1c & lipids, foot risk, retinal screening, injectable technique. Respiratory: asthma & COPD review, inhaler technique, peak flow, QA spirometry, action & rescue plans, rehab referral.
• CVD, CKD & obesity: clinic & home BP monitoring, lifestyle intervention, register recall & coding. Immunisation: childhood schedule, flu & COVID campaigns, adult, opportunistic & travel vaccination; cold chain & wastage records.
• Cervical sample taking (NHSCSP), failsafe & non-responder follow-up; NHS Health Check (40-74), learning disability, child, Grenfell enhanced & new patient checks; brief intervention for smoking & weight.
• Dressings, leg ulcer & post-op wound care, suture & clip removal, ear care; minor injuries & ailments per protocol, escalate to GP.
• Venepuncture, urinalysis & UTI assessment, ECG, point-of-care testing.
• Routine contraception (pill, checks, injection) under PGD/PSD, emergency contraception, chlamydia & STI screening.
• SystmOne recording supporting QOF, IIF & enhanced services; recall, register validation & interpreting support; chaperoning, audit & SEAs.
• Practise within the NMC Code & your competence; maintain registration, revalidation & indemnity; work to protocols, PGDs or PSDs.

PRACTICE NURSE / NURSING ASSOCIATE

JOB OVERVIEW

We are looking for a Practice Nurse or a Nursing Associate to join our small, busy team in North Kensington.

This is a standalone nursing post. You will be the practice's nurse, working directly alongside our GPs, our practice pharmacist and our wider multidisciplinary colleagues. It suits someone who is confident working within their own scope of practice, comfortable asking for help early, and interested in shaping how a nursing service runs rather than inheriting one.

We will consider applications at either Registered Nurse or Nursing Associate level. The two registrations carry different responsibilities, set out below.

WORKING FOR OUR ORGANISATION

We are a single-site NHS practice in North Kensington, part of the Neohealth Primary Care Network and a teaching practice with Imperial College London. In July 2026 we moved into purpose-built premises on Kensal Road with eight consulting rooms and a dedicated treatment room.

The clinical team is three GPs led by our GP Principal, supported by a practice pharmacist and independent prescriber and a reception team trained as care navigators. As a teaching practice, we are further supported by GP registrars and doctors in training. We host visiting health visiting, district nursing, midwifery and mental health colleagues, the MSK team, and the John Hunter Clinic's contraception and sexual health service on Wednesdays. It is a small enough team that you will know everyone by name and large enough that you will not be working alone.

We serve one of the most diverse and materially deprived wards in London, including families affected by the Grenfell Tower fire. That shapes how we work. We do not ask patients to call back, we aim to respond to new requests within 48 hours, and we offer appointments by telephone, online and face to face. Consultations are often supported by professional interpreting. It asks a lot, and the people who do well here are the ones who find that motivating rather than draining.

MAIN DUTIES OF THE JOB

You will deliver the nursing element of general medical services: long-term condition review, immunisation, cervical screening, treatment room care, health checks and health promotion. You will work within your own competence and NMC scope of practice, support our Quality and Outcomes Framework and enhanced service commitments, and contribute to a service that meets the fundamental standards under the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

DETAILED JOB DESCRIPTION AND MAIN RESPONSIBILITIES

Long-term conditions

• Diabetes: annual and interim review, HbA1c and lipids, foot risk assessment, retinal screening uptake, injectable technique, self-management support.
• Respiratory: asthma and COPD review, inhaler technique, peak flow, quality-assured spirometry, action and rescue plans, pulmonary rehabilitation referral.
• Cardiovascular, CKD and obesity: clinic and home blood pressure monitoring, lifestyle intervention, register recall and coding, weight management support.

Immunisation

• Childhood schedule, seasonal influenza and COVID-19 campaigns, adult and opportunistic vaccination (pneumococcal, shingles, pertussis in pregnancy, MenACWY, HPV).
• Travel health risk assessment and vaccination.
• Cold chain: fridge monitoring, stock rotation, excursion and wastage records.

Screening, health checks and prevention

• Cervical sample taking within the NHS Cervical Screening Programme, with failsafe and non-responder follow-up.
• NHS Health Check (40-74), learning disability, child, Grenfell enhanced and new patient health checks.
• Brief intervention and referral for smoking, weight, alcohol and physical activity, working with our smoking cessation adviser and social prescribing routes.

Treatment room and diagnostics

• Simple and complex dressings, leg ulcer and post-operative wound care, removal of sutures and clips, ear care where trained.
• Minor injuries and minor ailments within agreed protocols, escalating to a GP.
• Venepuncture and specimen handling, urinalysis and UTI assessment, 12-lead ECG, point-of-care testing.

Contraception and sexual health

• Delivered on site by the John Hunter Clinic (JHC@Canalside) on Wednesdays. You will identify need opportunistically, give general advice and refer in. This post does not carry a contraception caseload and LARC fitting is not part of the role.

Wider practice duties

• Chaperoning in line with practice policy.
• Accurate contemporaneous SystmOne recording using agreed templates and codes, supporting QOF, IIF and enhanced service claims.
• Recall, missed-appointment follow-up, register validation, and support to the Accessible Information Standard and interpreting arrangements.
• Contributing to significant event analysis, clinical audit and responses to patient feedback.

GOVERNANCE AND PROFESSIONAL RESPONSIBILITIES

• Practise within the NMC Code and the limits of your own competence, escalating promptly. Maintain registration, revalidation and indemnity.
• Work to practice protocols and to Patient Group Directions or Patient Specific Directions as applicable to your registration.
• Infection prevention and control: hand hygiene, aseptic technique, decontamination, sharps and clinical waste, and the annual IPC audit and statement.
• Maintain clinical rooms and equipment: stock, calibration records, emergency drugs, oxygen and resuscitation checks.
• Discharge safeguarding duties for children and adults at risk at the training level required for the role.
• Comply with UK GDPR and the Data Protection Act 2018, the Health and Safety at Work etc. Act 1974 and COSHH, and our equality, diversity and duty of candour policies.

SCOPE OF PRACTICE: REGISTERED NURSE AND NURSING ASSOCIATE

These are two distinct NMC registrations and the difference is not cosmetic. Please read this section before applying.

As a Registered Nurse you would assess undifferentiated presentations within practice protocols and your competence, supply and administer medicines under Patient Group Directions, immunise under PGD, take cervical samples once NHSCSP sample-taker trained, conclude long-term condition reviews and escalate treatment, and delegate to healthcare assistants.

As a Nursing Associate you would work to a plan of care set by the duty doctor. Nursing Associates may not operate under a Patient Group Direction. Every vaccination would need to be authorised in advance by a Patient Specific Direction issued by the duty doctor, who is also your supervising prescriber. Cervical sample taking would require written confirmation of eligibility from the local screening and immunisation team. You would contribute data collection and self-management support to long-term condition reviews, with the conclusion and any escalation made by the duty doctor.

• Qualifications
• Current NMC registration as a Registered Nurse (adult) or Registered Nursing Associate, with no restrictions on practice.
• Current revalidation and evidence of continuing professional development.
• Experience
• Post-registration clinical experience in an acute, community or primary care setting.
• Experience caring for a culturally and linguistically diverse population.
• Experience of working to written clinical protocols.
• Knowledge and skills
• Venepuncture, injection technique, blood pressure measurement, urinalysis and basic wound care.
