Main duties of the job
An exciting opportunity has arisen to join our multidisciplinary Primary Care Network team.
We are a dynamic and forward-thinking PCN, passionate about developing and delivering high-quality local services that meet the needs of our patient population.
We are looking to expand our clinical team by recruiting a General Practitioner to work collaboratively across our member practices and alongside our wider multidisciplinary PCN team.
This is an excellent opportunity for a GP who is keen to work within an innovative primary care environment, supporting the delivery and development of services across the PCN.
Job description
Clinical ResponsibilitiesIn accordance with the agreed PCN and practice timetable, the post-holder will undertake a range of clinical duties including:
- Undertaking face-to-face, telephone and, where appropriate, remote consultations with patients.
- Assessing, diagnosing and managing patients presenting with acute, chronic, undifferentiated and undiagnosed conditions.
- Making professional and autonomous clinical decisions regarding patients presenting directly or following referral from other healthcare professionals.
- Assessing the healthcare needs of patients and formulating appropriate management plans.
- Identifying disease risk factors and early signs of illness and providing appropriate preventative healthcare interventions.
- Developing and reviewing personalised care and treatment plans in consultation with patients and, where appropriate, their carers.
- Providing health promotion, education and appropriate counselling to patients.
- Initiating referrals and signposting patients to other health, social care, community and voluntary services where clinically appropriate.
- Admitting or discharging patients from the caseload and referring to other healthcare providers when appropriate.
- Undertaking home visits where clinically indicated and as agreed within the clinical rota.
- Participating in urgent, same-day and other clinical activity as required within the agreed PCN and practice rota.
- Reviewing and responding appropriately to clinical correspondence, investigations, test results and patient-related documentation.
- Maintaining accurate, clear and contemporaneous patient records in accordance with professional, regulatory and organisational standards.
- Ensuring appropriate and accurate SNOMED CT coding and recording of clinical activity within the electronic patient record.
- Collecting and recording relevant clinical data for audit, quality improvement and service evaluation purposes.
Prescribing and Medicines ManagementThe post-holder will:
- Prescribe medicines safely, appropriately and cost-effectively in accordance with current clinical guidance, local formularies and prescribing policies.
- Generate and authorise acute and repeat prescriptions as clinically appropriate.
- Review medication and respond to prescription and medicines-related queries in a timely manner.
- Consider medicines optimisation as part of the ongoing management of patients.
- Work collaboratively with PCN and practice pharmacists, pharmacy technicians and other clinicians to support safe prescribing and medicines management.
- Maintain awareness of relevant prescribing alerts, safety notices and local and national guidance.
PCN and Collaborative WorkingAs this is a PCN-wide role, the post-holder will be expected to:
- Work collaboratively across Kingswinford & Wordsley PCN member practices.
- Build effective working relationships with GPs, practice teams, PCN staff and the wider multidisciplinary team.
- Contribute to the delivery of consistent, high-quality care across the network.
- Participate in PCN-wide clinical services and initiatives.
- Contribute to collaborative projects and service development across the PCN.
- Support the development and implementation of new clinical pathways and models of care.
- Participate in multidisciplinary team meetings and collaborative case discussions where appropriate.
- Work closely with PCN pharmacists, nurses, allied health professionals and other members of the multidisciplinary team.
- Support coordinated working across primary, community and secondary care where appropriate.
- Contribute to the PCN's clinical, population health and service improvement priorities.
- Support quality improvement work and initiatives designed to improve patient outcomes and reduce variation across the network.
- Share clinical knowledge, learning and good practice across PCN member practices.
- Contribute positively to transformation and innovation within the PCN.
- Attend relevant PCN clinical, multidisciplinary, operational and educational meetings as required.
- Work flexibly across PCN member practices and services in accordance with operational and patient needs.
Clinical SystemsThe post-holder will be expected to be a competent and experienced user of EMIS Web.
Responsibilities will include:
- Using EMIS Web effectively and independently for patient consultations and clinical documentation.
- Recording accurate clinical histories, assessments, diagnoses and management plans.
- Using appropriate SNOMED CT clinical coding.
- Reviewing results, correspondence and patient records.
- Managing clinical tasks and workflow appropriately.
- Generating and authorising prescriptions.
- Completing appropriate referrals and clinical documentation.
- Working safely across different PCN practice organisations and EMIS organisational units where required.
- Maintaining accurate and timely electronic patient records.
- Using digital systems appropriately in accordance with information governance and confidentiality requirements.
- Using Microsoft applications, including Outlook and Teams and excel spreadsheets, effectively as part of day-to-day working.
Clinical Governance and Quality ImprovementThe post-holder will:
- Work in accordance with professional standards and all relevant PCN and member practice policies and procedures.
- Participate in clinical governance arrangements across the organisation.
- Contribute to clinical audit, quality improvement and clinical standard setting.
- Participate in significant event reviews and learning where appropriate.
- Identify and manage clinical risk appropriately.
- Contribute to the development and review of clinical pathways, protocols and procedures.
- Work in accordance with evidence-based practice and current national and local clinical guidance.
- Maintain awareness of relevant NICE guidance and other national clinical standards.
- Contribute to the effective use of clinical data to support service improvement and patient care.
- Support improvements in the quality and consistency of clinical recording and coding.
Professional ResponsibilitiesThe post-holder will:
- Maintain full professional registration and a licence to practise.
- Maintain appropriate inclusion on the NHS England Medical Performers List.
- Maintain compliance with appraisal and revalidation requirements.

- Access patient information only where there is a legitimate clinical or professional requirement.
- Ensure clinical information is recorded accurately and securely.
- Follow PCN and practice policies regarding confidentiality, information security and records management.
SafeguardingThe post-holder will:
- Understand their responsibilities for safeguarding children and adults at risk.
- Recognise potential safeguarding concerns and act in accordance with local and national procedures.
- Maintain appropriate safeguarding training for their role.
- Work collaboratively with relevant professionals and agencies where safeguarding concerns arise.

This role exists to support the delivery of the Kingswinford & Wordsley Primary Care Network agenda. The post will form an integral part of the PCN leadership team working alongside the Practice Managers and the Clinical Director and the PCN Partners.
