Job summary

<p><strong>Help patients get home safely and sooner by keeping our wards moving.</strong></p><p>We are looking for a motivated and <span class="single_desc_int_gb_word">compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team, based on <strong>Swindon Intermediate Care Centre</strong><strong>.</strong> This is a rewarding opportunity to make a real difference to patients and their families by <span class="single_desc_int_gb_word">supporting safe, timely discharges into community services across our BSW Community Hospitals.</span></span></p><p>In this role, you will work closely with nurses, therapists, social workers and other members of the multidisciplinary team to coordinate patient flow and ensure everyone has the right information at the right time. You will play a key role in <span class="single_desc_int_gb_word">supporting smooth discharge processes, maintaining accurate records, completing referrals and ensuring clear communication across services</span>.</p><p>You will also build strong, <span class="single_desc_int_gb_word">supportive relationships with patients and their families on the ward, helping them to <span class="single_desc_int_gb_word">understand the discharge process and keeping them informed throughout their hospital stay</span></sp
