We have an exciting opportunity for an Advanced Nurse Practitioner. Our ideal candidate will have experience of working within Primary Care and will be able to work autonomously seeing patients.

The number of hours for this role is flexible, part time applications are welcome. Additional shifts can be offered as part of our extended hours.The successful applicant will benefit from opportunities for further training and development are encouraged and supported.

As a minimum you will be an independent non-medical prescriber with level 7 clinical diagnostic and assessment qualifications having experience of working in an urgent or primary care setting.

Main duties of the job

You will undertake comprehensive health assessments both face to face and over the telephone, including adults (and paediatrics); practicing autonomously whilst demonstrating a person-centred approach to care delivery.

• Treat patients who request same day appointments, as well as carrying out some reviews for those requiring ongoing care, liaising with the GPs
• Ensure clinical practice is safe and effective and remains within boundaries of competence
• Assess, diagnose, plan, and treat patients with minor illnesses with appropriate referrals to GPs and other clinicians and prioritising their workload effectively.
• Refer for investigations and ongoing emergency care, i.e., referral to hospitals in the appropriate manner.
• Must be capable of providing care to all patient groups autonomously and effectively and without discrimination
• Maintain membership of a regulated body.
• Assist the Lead Nurse to provide leadership and management of the Nursing Team including quality governance, and learning.
• Provide support to the Duty Doctor where required, for effective and timely management of on the day telephone calls.
• Undertake Home Visits, alongside the GPs who will provide support if required
• Establish working relationships with Primary Care Teams and MDT's.

Job description

The Advanced Nurse Practitioner(ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

• Assess, diagnose, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis or complex needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care.
• Assess, diagnose, and manage minor illness, acute and chronic conditions, integrating non-medical prescribing, non-pharmacological and multi-disciplinary resources.
• Work with patients to support compliance with and adherence to prescribed treatments.
• Work autonomously to provide a mix of face to face and telephone appointments.
• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
• Review Long Term Conditions, including shared decisions for an agreed management plan, and optimising medication.
• Provide advice, assessment and treatment plans health and well-being.
• Assist the Lead Nurse to provide leadership and management of the Nursing Team including quality governance, and learning.

St Augustine's Medical Practice is based in the new Chocolate Quarter development in Keynsham, we have branch surgeries in nearby Saltford and Temple House at Keynsham Health Centre.

Our purpose built modern spacious premises, easily commutable from Bath/Bristol and surrounding areas and a short walk from Keynsham train station / high street. Our friendly nursing team of ANP's, Practice Nurses and HCAs is part of our diverse and changing workforce, which consists of GPs, FCP's , paramedics, pharmacists, health care assistants, and other allied health professionals.

We have regular meetings to manage and continually improve our patient care. We are passionate about training the next generation of workforce having been teaching practice for many years.

We are part of the Keynsham Primary Care Network (PCN) which includes West View. Working together allows us to take on bigger projects that we couldn't do alone, share good practice and improve care for patients.
