Overview of Role: We are looking for individuals with a passion and enthusiasm to work with some of the most vulnerable and complex people within our community. The right candidate should be approachable and display excellent communication skills, compassion and a non-judgemental attitude. We are looking for a Health care assistant to join the team on a bank basis, covering all the services within Inclusion Health.

Main duties of the job

Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.

Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.

Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.

The targeted demand management service offers a wound care service in Fleetwood on weekly basis, and a health care assistant led health check service every two weeks.

Job description

Day to Day Duties to include, but not exhaustive:

Support the Management team to ensure all QOF Targets are met

Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures

Prioritise, organise, and manage own workload in a manner that maintains and promotes quality

Deliver care according to local and national guidelines

Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making

Work within all relevant policies and procedural guidelines e.g., infection control, chaperoning and risk management

Safeguarding responsibilities for adults and vulnerable groups, reporting concerns appropriately at the correct time and engaging with services as needed.

Clinical practice:

To support qualified clinicians in supporting patients seen within the Complex Lives services

To provide advice for patients with presenting conditions with support of qualified clinicians

To provide Wound Care, undertake ECGS, Observations and Venepuncture to support the trained clinicians.

Patient education and health promotion

Work to attain the service KPIs and targets.

Ensure all patient contact is recorded in a timely manner on the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service.

To ensure mandatory training is kept up to date.

Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language, and emotion.

Flexibility in attending organised educational updates and eagerness to expand on current knowledge base and cascading new educational information to colleagues whenever possible.

Inform the Clinical Manager of any untoward incidents Work within all relevant policies and procedural guidelines e.g., infection control, safeguarding, chaperoning and risk management.
