Job summary

An exciting opportunity has arisen for a compassionate and motivated NMC registered Mental Health Nurse to join our Community Mental Health Service in Bridgwater on a 12 month secondment or fixed term contract.

This role offers the chance to work within a supportive multidisciplinary team, delivering high-quality care to people with severe and enduring mental health difficulties. The successful candidate will support a number of specialist clinics, including the Depot Clinic, Clozapine Clinic and Wellbeing Clinic, providing medication administration, physical health monitoring and ongoing mental health support.

Alongside clinic responsibilities, the post holder will play an active role in the team's duty system, supporting assessments, responding to urgent referrals and contributing to the delivery of recovery-focused care across the wider Community Mental Health Team.

Due to the community-based nature of the role, applicants must hold current NMC registration and be able to travel independently across the Bridgwater locality

Main duties of the job

  • Support the delivery of the Depot Clinic, Clozapine Clinic and Wellbeing Clinic, ensuring safe administration and monitoring of prescribed medications.
  • Monitor service users' mental and physical health, escalating concerns appropriately and supporting positive health outcomes.
  • Undertake mental health assessments and contribute to the development, implementation and review of recovery-focused care plans.
  • Participate in the team's daily duty function, responding to urgent referrals, risk concerns and service user enquiries.
  • Support the wider Community Mental Health Team with assessments, care coordination and clinical interventions.
  • Develop and maintain therapeutic relationships with service users, carers and partner agencies to promote recovery and independence.
  • Assess and manage risk, contributing to effective risk management and safeguarding processes.
  • Maintain accurate clinical records and contribute to service development, audit and quality improvement activities.
  • Work collaboratively with colleagues across health, social care and voluntary sector organisations to ensure coordinated care.
