Job summary

CYPMHS South Hertfordshire is seeking to recruit a highly skilled and motivated Children and Young People's Social Worker with an appropriate professional qualification and relevant experience to join our multidisciplinary team.This is a full-time position (37.5 hours per week) offered on a permanent basis.The successful candidate will play a key role in delivering high-quality care and will make a significant contribution to the clinical and social work provision within the team. The post holder will actively participate in referral screening and allocation processes, undertake comprehensive assessments, provide evidence-based interventions, coordinate care, manage risk, and promote the safeguarding of children, young people, and vulnerable adults. The role also includes contributing to multidisciplinary case discussions and service development activities.In addition, the post holder will provide consultation, advice, and supervision to colleagues, students, and professionals within partner agencies and services.

Main duties of the job

  • To develop and deliver highly specialist, evidence-based interventions for children, young people, and their families, drawing upon a wide range of theoretical frameworks and treatment approaches.
  • To provide timely, accessible, and person-centred services through both clinic-based and community outreach models.
  • To offer highly specialist advice, consultation, and formulation-based guidance to colleagues and partner agencies, working autonomously within professional and organisational guidelines.
  • To care coordinate a caseload of service users, undertaking highly specialist assessments and delivering integrated packages of care.
  • To contribute to the continued development of social work practice within the service, including the provision of supervision to social work colleagues and professionals from other disciplines.
  • To actively participate in clinical supervision, appraisal processes, and continuing professional development activities to maintain and enhance professional competence.
  • To chair and attend multidisciplinary team meetings and provide appropriate clinical and professional input to decision-making processes.
  • To work collaboratively with colleagues across HPFT and partner organisations, providing specialist social work consultation to support the early identification, assessment, and management of mental health difficulties, neurodevelopmental conditions, and associated risks.

Job description

The post holder will take responsibility for making a significant contribution to the clinical social work provision within CYPMHS South Hertfordshire. This will include active participation in referral processes, assessment, treatment planning and delivery, risk management, safeguarding children and vulnerable adults, care coordination, and contributing to multidisciplinary case discussions and service-related meetings.

The successful candidate will be expected to provide consultation and advice to colleagues, students, and professionals working across partner services. They will support the ongoing development of social work practice within the team and contribute to service improvement initiatives, ensuring the delivery of high-quality, evidence-based care that meets the needs of children, young people, and their families.
