Job summary

An exciting opportunity has arisen for a Band 5 Mental Health Nurse to join our Runnymede Neighbourhood Mental Health service.

We are looking for a passionate nurse to join our Frailty, Later Life & Dementia community service who will focus on providing support and high-quality mental health nursing care for clients and their carers, targeting and prioritizing people with severe mental health problems in later life and or dementia.

You will manage a caseload of clients, and use evidence based/client centered principles to assess, plan, implement and evaluate interventions as part of the holistic approach. Excellent communication skills are essential, working alongside the MDT, working closely and collaborating effectively with all agencies and partner organizations involved in the care of our clients.

In return you will have a fulfilling and varied role within a friendly and supportive team. You will receive active management support and opportunities for personal and professional development.

This job is suitable for someone who is passionate about providing a high quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.

Main duties of the job

The post holder will maintain client confidentiality, work closely with external agencies, and follow Trust and service policies and procedures. They will accurately record information, work safely and independently when required, support the team, and contribute to a safe working environment. The role also involves undertaking training and professional development, participating in performance reviews, and maintaining a good understanding of safeguarding responsibilities for both adults and children.
