Job summary

You'll support people receiving rehabilitation care, helping them maintain their wellbeing, independence and quality of life. Working as part of a caring and supportive team, you'll help deliver person-centred care that focuses on what matters most to each individual. Your contribution will make a real difference to patients' experiences by providing practical support, encouragement and compassionate care every day.

Main duties of the job

What you'll do:

  • Support patients with care activities in line with their individual care plans
  • Carry out agreed clinical tasks, such as observations, blood pressure monitoring and specimen collection following training and competency assessment
  • Communicate effectively with patients, carers and healthcare professionals
  • Record patient information accurately on electronic systems and support basic administrative tasks
  • Help maintain a safe, organised environment, including preparing equipment and managing stock levels

What we're looking for:

  • A Level 2 qualification in Adult Health and Social Care, or equivalent experience
  • Experience supporting people in a healthcare, homecare or care home setting
  • Strong communication and interpersonal skills, with a caring and respectful approach
  • Ability to work independently and as part of a team
  • Willingness to complete the Care Certificate during induction if not already achieved
