Job summary

We are pleased to offer an exciting opportunity for ahighly motivated and enthusiastic Advanced Clinical Practitioner (ACP)to join ourHome Visiting Service.

The successful candidate will provide high-quality, patient-centred care to patients in their own homes, working autonomously as part of a multidisciplinary team.

Differing hours will be considered.

Main duties of the job

The role will support a core team of ACPs across our PCN North Hardwick and Bolsover providing home visits for 7 practices

As an ACP you will:

• Provide clinical leadership, including direct clinical care, and advice on complex issues
• Establish a cohesive team approach for the ACP workforce development
• Support the home visiting service leads and managers with audit, innovation, and performance management
• Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
• Assist in feedback for staff appraisals.
• Deputise for the CL where necessary, if not being carried out by other members of staff.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to adults and children, presenting with undifferentiated diagnosis

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults and children.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Were seeking an ACP To provide leadership support for the clinicians already working in the service, to advance the care of the presenting patients (in line with our 4 pillars approach).

Were looking for ACPs with a proven track record of working at this level in an urgent care environment and who can support Service Leads and Unit managers in safe care delivery.
