Job summary

To deliver NHS Health Checks to eligible Sutton patients within Enhanced Access clinics, undertaking clinical assessments, blood tests, accurate documentation, patient education, and follow-up in line with NHS Health Check guidance, Sutton PCN protocols, and NICE guidelines.

Main duties of the job

Deliver NHS Health Checks to eligible patients in line with the NHS health check programme

Undertake face to face assessments, including:

o Height, weight, BMI and waist circumference

o Blood pressure measurement

o Lifestyle assessment (smoking, alcohol, physical activity, diet, QRISK,QDiabetes)

o Blood test (e.g. cholesterol, HbA1c)

Complete follow up telephone appointments

Accurately record all activity on the clinical system EMIS

Calculate and explain CVD risk scores in a clear and supportive way

Provide brief advice and signposting to lifestyle services

Escalate or refer patients appropriately in line with PCN protocols

Ensure informed consent, confidentiality, and data quality standards are met

Work across multiple practices as required by the PCN

Adhere to infection prevention, safeguarding, and information governance

requirements

Deliver bloods to hospital at the end of each clinic.

Participate in Health care assistant meetings to support the development.

Identify abnormal results (e.g. raised blood pressure, cholesterol, HbA1c) and

escalate appropriately in line with local protocols.

About us

Sutton Primary Care Networks is the collaborative vehicle for the four Sutton PCNs and the former Sutton GP Federation, working together as a single entity. We are a Community Interest Company that aims to reinvest surpluses back into general practice and primary care for the benefit of our practices and the people who form the communities we serve.

Sutton has four PCNs whose boundaries are based on historical relationships and organisational groupings. There are some similarities in residents' health and care needs that are unique to a particular PCN, and many more that are shared across all four PCNs.

Our four PCNs represent all 21 GP surgeries in Sutton. Their collective footprints cover the London Borough of Sutton and its residents, who can access and benefit from the services and improvements offered by Primary Care Networks.

Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with all general practices being required to be in a network by June 2019, and CCGs/Integrated Care Boards (ICBs) being required to commit recurrent funding to develop and maintain them.

The networks will have expanded neighbourhood teams comprising a range of staff, including GPs, pharmacists, district nurses, community geriatricians, dementia workers, and Allied Health Professionals such as physiotherapists and podiatrists/chiropodists, alongside social care and the voluntary sector.
