The successful candidate will be an experienced Practice Nurse with a strong interest in the management of long-term conditions.

Working closely with nurses, GPs, pharmacists, and the wider multidisciplinary team, the post holder will support the delivery of evidence-based care for patients with long-term conditions, working alongside the practice's clinical leads to ensure effective and proactive management in line with current clinical guidelines.

Main duties of the job

The role will include core practice nursing duties such as immunisations, vaccinations, screening, and health promotion, the principal focus will be the assessment, review, and ongoing management of patients with chronic diseases.

Contributing to and maintaining systems that support chronic disease management, promoting positive health outcomes, and providing care to housebound patients where required.

Job description

The following are the core responsibilities of the Practice Nurse. There may, on occasion, be a requirement to carry out other tasks, depending on factors such as workload and staffing levels:

a. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance.

b. To assess the needs of patients, ensuring the provision of healthcare is appropriate and incorporates evidence-based practice.

c. To develop, implement and embed health promotion and wellbeing programmes.

d. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.

e. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.

f. As required, to provide routine nursing care to patients in accordance with clinical evidence, NICE guidance and local policies.

g. To administer medication to patients in accordance with PGDs and own scope of practice.

h. To request pathology services as required.

i. To ensure the provision of chronic disease management, delivering patient care as necessary and referring patients to specialist care as required.

j. To maintain accurate clinical records in accordance with existing legislation.

k. To ensure the appropriate maintenance and use of chronic disease registers.

l. To ensure compliance with the practice chaperone policy.

m. To prioritise health issues and intervene appropriately.

n. To support the team in dealing with clinical emergencies.

o. To participate in vaccination programmes for adults and children.

p. To support patients in the use of their prescribed or over-the -counter medicines, within own scope of practice.

q. To liaise with external services and agencies to ensure vulnerable and other patients are supported appropriately.

r. To delegate clinical responsibilities appropriately, ensuring safe practice and that tasks remain within the delegates scope of practice.

s. To support the clinical team with all safeguarding matters, in accordance with local and national policies.

t. To understand practice and local policies on substance abuse and addictive behavior, referring patients appropriately.

u. To deliver opportunistic health promotion where appropriate.
