Job summary

We are excited to offer an excellent opportunity for an experienced and enthusiastic HF CNS to join our heart failure service for a 12 month fixed term/secondment as a Band 7 Heart Failure CNS. This post offers the chance to be part of a proactive, multidisciplinary team that delivers high-quality, patient-centred care across both hospital and outpatient settings. You will be based at Heartlands, Goodhope and Solihull Hospitals and play a key role in the assessment, management, and ongoing support of adults living with heart failure.

As a HF CNS, you core aspect of your role will be patient education, prescribing heart failure treatments, creating treatment plans and arranging follow-up following both inpatient and outpatient reviews, in collaboration with cardiologists, GPs, and wider primary care and community teams.

We are looking for a Registered Nurse with post-registration experience managing heart failure, are an NMP, and have health assessment qualification. You should have, or be working towards, a relevant specialist qualification such as an MSc module in heart failure. Strong clinical decision-making, communication skills, and the ability to work both independently and as part of a wider MDT are essential.
