The role includes working with individuals to understand their health needs and to plan appropriately for these. The role will cover holistic care which includes both physical and mental health aspects. Case management and coordination of support, assessing and reviewing the needs in a person centred approach. Liaising with other agencies and providers to ensure reasonable adjustments are in place.

Main duties of the job

The Community Nurse is responsible for assessing, planning, coordinating and reviewing a caseload of adults with learning disabilities. They may have either or both physical and mental health care needs. This will include relationships with Primary and Secondary health care, as well as private and voluntary care agencies.

To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high-quality outcome focused service in a person-centred approach.

This is a generic community care role providing support and cover across Plymouth city localities.

There is responsibility for participating in reviewing a persons eligibility for Continuing Healthcare funding and is responsible for making consistent and well-informed professional decisions about the determination of CHC funding, ensuring that the national framework is applied, and they adhere to all local policies and procedures.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Job description

Clinical Responsibilities

Participating in the undertaking of health needs reviews and holistic health assessments to identify needs and plan care accordingly.

To identify, plan and manage risk of an individuals care needs, relating to physical health, behavioural and/or mental health.

To be responsible for assessing, planning and making recommendations for future care in partnership with patient, carer and multi-professional agencies including case conferences for reviewing the packages of care and care co-coordinating as appropriate.

To liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.

To ensure consideration of Mental Capacity in Best Interest decision making.

To maintain effective and appropriate communication networks with members of the multi-disciplinary team, the independent sector homes and all stakeholder agencies and organisations to ensure that the individual care needs can be met.

To contribute to multi-disciplinary team specialist assessments.

Ability to work as an autonomous practitioner, planning, prioritising and organising workload according to the needs of the service.

To provide any relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.

To provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.

To act as a role model for other staff and students demonstration high standards of practice and professional conduct.

To undertake health promotion/education whenever possible to all adults with a Learning Disability and also to contribute to planned health promotion activities.

To work with all other agencies with regards to the Safeguarding process.

Managerial Responsibilities

To be responsible for coordinating reviews as per guidelines.

Prioritise caseload to meet competing demands

To be aware of and abide by the Livewell SouthWest policies, standards and Clinical Governance initiatives.

Present confidentiality and be aware of the Data Protection Act, access to health records and consent for assessment process.

Involvement in continual audit and review of services to determine quality.

Maintain safe record-keeping and confidential practice, in line with Livewell SouthWest and professional guidance and be aware of the Data Protection Act, access to health records and consent for assessment process.

To ensure electronic record keeping policy is adhered to.

Professional Responsibilities

To maintain a high professional standard of conduct at all times in accordance with NMC/ professional bodies code of professional conduct. Maintain an active status on NMC/ professional bodies register.

To maintain own re-validation portfolio.

Assume responsibility for own professional development and personal knowledge. Undertake and contribute to research where appropriate.

Monitor own performance against agreed objectives through the process of appraisal and personal development plans and professional body regulations.

Maintain professional expertise by arranging and attending meetings, study days and mandatory training and service training.

Plan and participate in the supervision and provision of experience for all nurse learners and other personnel as appropriate.

Participate in regular line management, caseload management and clinical supervision.

To deliver appropriate training and subsequent support to other staff, students and outside agencies when required.

5. COMMUNICATIONS AND RELATIONSHIPS

The post holder will be required to work within the MDT function of the CLDT and may need to be part of wider MDT groups relating to client need.

The post holder will liaise between other health, social and educational services in assessing, planning and reviewing client care needs/service provision.

The post holder will also work with other voluntary and secondary care providers.

7. ADDITIONAL INFORMATION FOR ALL POSTS

The post holder is required to comply with all relevant policies and procedures pertinent to their post. Current versions can be found on the intranet or via your manager. The areas listed below are those Livewell Southwest currently places particular emphasis on. Failure to follow correct policies and procedures may result in disciplinary action.

7.1 Risk Management

In accordance with the Risk Management Strategy, employees will participate, whenever required, with the risk management process. They will support line managers by attending mandatory and statutory training, completing incident/accident forms for every adverse event or near miss that occurs, report all defects and complaints, and communicate any dangerous situation to individuals potentially at risk.

7.2 Health and Safety at Work

You must co-operate with those in authority and others in meeting the statutory requirements and in following policies and procedures. A copy of the Health and Safety Policy is available from the intranet or from the Risk Management Department.

You are reminded that in accordance with the Health and Safety at Work Act 1974 you have a duty to take reasonable care to avoid injury to yourself and to others affected by your work activities.

You will be notified where your post carries a requirement for immunisation.

You may be required to be able to undertake physical intervention training and participate in physical intervention as part of a physical intervention team and Basic Life Support (BLS).

7.3 Infection Control

Livewell Southwest is determined to eradicate healthcare-acquired infection and puts a great deal of emphasis on the responsibility of all staff to ensure their own personal and others compliance with Infection Control (including Hand Washing) Policies. All staff must comply with infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate their concerns to the Director of Operations or Chief Executives Office).

7.4 Safeguarding Children and Adults

All employees have a duty to safeguard and promote the welfare of children and adults and are required to act in such a way that at all times safeguards their health and wellbeing. Familiarisation with and adherence to national and local safeguarding adults and children policies is an essential requirement upon all employees. Livewell Southwest has specific safeguarding policies and in addition, employees also have a responsibility to practice and work within the multi-agency policy developed by the Safeguarding Adults Board and the Safeguarding Children Board. Staff are also required to participate in related mandatory/statutory training.

7.5 Research

For clinical posts at bands 5 and 6, there is an expectation that engagement in research will be part of this role. Therefore, an awareness of the value and relevance of research is expected and the post holder should either support existing research within their specialty area as appropriate or identify opportunities to raise awareness of research possibilities. For clinical posts at band 7 and above, the post holder will be required to actively participate in complex audits using research methodology, or participate as required in clinical trials or equipment testing, and will demonstrate high level involvement in local ongoing research projects.

7.6 Sustainability and climate change

All staff are expected to take responsibility for the reduction of carbon emissions within their area of the organisation. In particular this may relate to reducing energy consumption, making low carbon travel choices, consideration of goods and services being purchased, and waste reduction.

7.7 Other

This Job Description is not exhaustive and may change as the post develops, but such change will not take place without consultation between the post holder and their manager. Job descriptions should be reviewed at least annually at the appraisal meeting.

The Working Time Regulations apply to all employees of Livewell Southwest. In particular, Livewell Southwest will not permit staff in all employments to work in excess of 48 hours in any one week except where there are exceptional service needs where an absolute limit of averaging over a reference period of 17 weeks would apply.
