Job summary

This is an exciting opportunity to join our friendly Community Learning Disability Team in North Tyneside. Our team is a large well established and supportive multi-disciplinary team offering holistic health care to children and adults who have a learning disability living in North Tyneside.

The ideal candidate will have an understanding of National and Local Policies relating to people with learning disabilities, experience of working with people who present with challenging behaviour as well as experience of working within an PBSframework and the ability to work effectively with children and their families/carers.

This is a new post, the successful candidate will join our already well established childrens workstream to carry out ADHD assessments for children who have a learning disability.

Main duties of the job

The post-holder will work as part of a multi-disciplinary team and will carry out ADHD diagnostic assessments for children who have a learning disability, with appropriate training available to the successful applicant.

The post-holder will have opportunities to learn about the wider roles and responsibilities of the Community Learning Disability team and to support the Duty system.

Opportunities to undertake service development project will be available the post-holder will provide support, training and supervision to others.

The post holder will be welcomed as a member of the friendly, community learning disabilities team in Northumbria Health Care Trust offering regular team meetings, robust support and supervision, and a commitment to continued professional development.

As an integral part of North Tyneside Community Learning Disability Team, the post holder will be solely responsible for a personal caseload including assessment, planning, implementing and evaluation of care.They will be passionate about working in collaboration with service users, their family, carers and professional colleagues.

Job description

  • Carry out ADHD diagnostic assessments for children who have a learning disability.
  • To provide a high quality community nursing service to adults and children who have a learning disability within the North Tyneside area.
  • To work across Primary and secondary healthcare services to ensure equality of access for those with learning disabilities.
  • To work with other agencies and services (statutory and non-statutory) to ensure equality of access for those with a learning disability and facilitate effective communication between clients and other agencies.
  • To provide community nursing support to adults and children who have behavioural/mental health problems. Support the design and implement Positive Behaviour Support Plans to provide intensive, proactive support and crisis intervention to patients, carers and care providers to prevent/ reduce admission to in-patient units and to maintain community placements.
