Job summary

An opportunity has arisen for an enthusiastic and highly motivated youth worker to join the innovative Barts Health NHS Trust High Risk Young Adult Service. This mobile service will comprise a Clinical psychologist, Social workers Youth Workers supporting clinical teams looking after young people living with a long-term health condition.

These exciting new posts have been developed to improve care and outcomes for young people (age 16-25) living with a long-term health condition across North East London. The post holder will support young adults and their families who may be struggling with a range of difficulties related to living with diabetes, as they make the transition from paediatric care to adult services. The work will mainly be with young adults living with diabetes, heart disease and kidney disease.

As these are new posts, the post holders will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users, and to take a leading role in the future of young people services across North East London.

The post will involve cross site working across Barts health and North East London communities: Mile End Hospital, Whipps Cross Hospital, the Royal London Hospital, and St Bartholomews Hospital and Homerton.

This varied post will involve engaging and supporting young people (age 16-25) and their families, advocating for them, and facilitating their access to medical care.

Main duties of the job

To provide targeted support to young people living with a long-term condition, supporting them to have an increasing role in managing their own care. To participate in the multi-disciplinary team using specific skills to contribute to the care and understanding of the young person, their families and carers. To encourage and help to develop a peer group network of support.
