Job summary
An exciting opportunity has arisen for a skilled, enthusiastic and motivated Assistant Practitioner to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.
Working within a dynamic multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Administrative Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.
As part of the UCR service, you will support clinicians in responding to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery and safe assistance from the floor.
The successful candidate will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes to adults with physical and mental health needs.
Main duties of the job
The successful candidates will be expected to work holistically alongside the multidisciplinary team. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.
You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.
The ideal candidate would be someone with experience of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients in their own homes.

If you do not hold the Foundation degree you will be supported to enrol on the Assistant Practitioner foundation degree to aid your future development.

Job Purpose
To work alongside Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Support Workers to provide assessment and treatment intervention to people within the Nottingham City Community, who are at risk of hospital admission or to facilitate discharge from hospital.
To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values. You will be working in a variety of settings.
You will participate actively in the services training strategy, including in service training and multi-agency training.

Key Responsibilities
- Management & Leadership
- To provide support to other Assistant Practitioners as appropriate.
Clinical Skills
- To undertake individual treatment and interventions with clients under the supervision of qualified staff.
- to refer service users to external agencies and specialists when their needs are beyond the assistant practitioner role (See A.P guidelines)
- To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.
- To promote wellbeing, safe mobility, transfers and effective communication.
- To provide instruction, advice and intervention to service users and carers in line with the multi disciplinary team guidelines, (see A.P guidelines).
- To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence. (See A.P guidelines)
- To assess for and commission a range of adaptations in line with the multi disciplinary team guidelines (see A.P guidelines). To attend and contribute to team meetings and case discussions using effective communication.
- To maintain comprehensive and accurate records in line with employers polices To promote health and wellbeing with all citizens To undertake basic wound care tasks as delegated by the District Nurse.
- To undertake a range of nursing competencies, as per the AP competency framework.

- To be actively involved in Clinical Governance, implement policies and proposing practice changes.
- To comply with organisations Lone Working Policy and local team guidelines at all times when working alone.
Planning and Organisation
- To be responsible for own time and caseload management.
- To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care.
- To use information technology on a daily basis to record information and statistics, to aid communication and produce reports.
- To ensure the Health and Safety of staff and clients at all times in accordance with Health and Safety legislation, Trust policy and procedures.
- To undertake any other duties which may be reasonably required.
Training
- To comply with and complete statutory and mandatory training in line with Nottingham CityCare Partnership policy.
- To attend training events as appropriate to support your continuing professional development.
- To maintain a continuing professional development portfolio identifying learning objectives.
- To actively participate in your own supervision, annual performance review and personal development plan.
