Job summary

To support the delivery of high-quality NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC) by undertaking and contributing to all review of care packages for individuals with complex health needs under the National Framework for NHS Continuing Healthcare.

The role involves conducting review including FNC reviews, CHC reviews, including Personal Health Budget reviews and identifying eligibility changes, ensuring that individuals receive appropriate, safe, and high-quality care.

The post holder will undertake these reviews, and liaise with providers, and ensure compliance with national policy, safeguarding standards, and person-centred care.

To prepare/complete appropriate documentation/tools to enable the level of NHS funding to be determined within the parameters of the National Framework for Continuing Health Care and Funded Nursing Care and Joint Funding arrangements locally.

Main duties of the job

• Co-ordinating reviews for NHS Funded Nursing Care and Continuing Health Care
• Review on-going of individuals eligible for CHC, NHS funded care in accordance with National Framework for NHS Funded Care, and Joint funded care packages of care.
• Providing coordination of, and participating in, meetings and providing information, advice and support as required
• To actively promote communication and partnership working between the statutory, independent and voluntary sectors.
• To communicate relevant information and to provide advice and support on funded nursing/continuing health care to service users and carers.
• To liaise with other health professionals on clinical matters as part of the assessment process.
• Complete Continuing Healthcare reviews in line with National Guidance for Funded NHS Nursing Care and the National Framework for Continuing Healthcare
• Recognise changes in a patient's care needs and refer to senior assessors for further assessment
• Ensure care plans, risk assessments, and documentation areaccurate, up-to-date, and meet quality standards; provide feedback to care providers when improvements are needed
• Liaise with the wider multidisciplinary team (MDT) to gather relevant assessments and updates before conducting reviews

Job description

• Escalate cases to Band 6/7 staff
• Follow up on requested improvements by care providers and escalate any unresolved concerns to the Continuing Healthcare Team, Safeguarding Lead, or Quality Assurance Manager
• Serve as a patient advocate, delivering sensitive information and supporting clients and their representatives regarding FNC/CHC processes
• Managing a caseload of patients
• Supporting investigations into an individuals eligibility for NHS Continuing Healthcare following an individual challenge or dispute

There is an expectation of 40% on patch working, with office bases at County Hall, Glenfield, Leicester and Haylock House, Kettering. Staff can choose which office is their primary base for contractual purposes and travel claims.

This role will require a minimum of 2 days 'on patch' working in Leicestershire, Northamptonshire and Rutland in our 2 main offices or partner sites
