Job summary

We have an exciting opportunity to recruit 2 band 4 Cancer Care Coordinators to work alongside our Colorectal cancer specialist nursing teams providing non-clinical support. These exciting role will underpin the delivery of the personalised care agenda to people affected by colorectal cancer.

One post will be based in Darlington Hospital and one in Durham Hospital.

We are looking for highly motivated individuals who have excellent communication skills, are empathathetic, excellent problem solvers, and have a passion for delivering high quality person-centred care. You will provide the nursing team with administrative support, alongside non-clinical support and signposting for patients.

You will be qualified to at least NVQ Level 3 or have equivalent experience, possess good IT skills and have experience working in a healthcare setting. You will also be able to demonstrate resilience, compassion and a willingness to embrace innovation.

Main duties of the job

You will be based with the colorectal cancer specialist nursing team where you will assist with the day to day running of the service. This includes:

• Supporting the Multi Disciplinary Team meetings and outcomes to ensure the timely management of patients on a cancer pathway.
• Delivering personalised care through the use of Holistic Needs Assessments, health & wellbeing offers, Patient Statified Follow Up (PSFU), treatment summaries, referals to support services, and close working with the Macmillan Information and Support Team.
• Liaising with community and primary care teams to facilitate the timely management and treatment for patients.
• Advocating for patients, helping them navigate health & social care while empowering them to make the best choices for their needs.
• Providing support to those who may be upset or distressed.
• Considering any support needed in relation to health literacy and health inequalities.
• Dealing with enquiries from people dealing with cancer, these may be face to face, over the phone or by email.
• Visiting hospital departments to offer support to patients or family members as needed.

Some travel across sites is expected.

Job description

You will work as part of the colorectal cancer specialist nursing team and the wider Cancer Services team to support the delivery of safe, compassionate and joined-up care. In this non-clinical role, you will work under the supervision of the Cancer Clinical Nurse Specialist team to assist in the provision of care. You may have some direct patient contact and your role will make a valuable contribution to improving the experience of people living with cancer.

You will contribute to Cancer Waiting Times (CWT) performance by supporting Clinical Nurse Specialists and patients throughout the pathway, helping to ensure appointments, investigations, and treatments are coordinated efficiently and delays are minimised.

You will be a flexible and adaptable advocate for patients, helping them to navigate health and social care while empowering them to make the best choices for their needs. You will possess excellent communication skills and will provide support to those who may be upset or distressed.

Working collaboratively with a wide range of services, you will ensure that people affected by cancer can access support in their local community, for example Citizens Advice, Joining The Dots, Social Prescribing Link Workers, Health Coaches and hospices.
