We're looking for a Social Prescriber and Recovery worker to join our Autism and Learning Disability Liaison team.

The successful candidate will be supporting in the implementation of programmes of care, under the supervision of the Care Navigators. The Social Prescriber and Recovery Worker will be working across health, social care, education, and the voluntary sector with families, to ensure children and young people with the most complex needs receive appropriate personalised and crucially, integrated support

A typical day will be very varied and very rewarding. This role provides the opportunity to make a profound impact to children, young people and families in accessing support and provisions within the community in a timely way.

Main duties of the job

The Social Prescriber will support the whole 'System' to deliver high quality, effective and efficient care and aim to:

• Improve the experience of children & young people accessing Health, Social Care, Education, and voluntary services
• Supporting the Care Navigators in delivering support and signposting.
• Improve family and CYP access to community-based support.
• Support improvement in the quality of care and provision available and delivered to the individual and family
• Lead on developing a clinical resource folder, that is maintained and updated by the social prescriber. This folder will hold up to date information about services, resources, charities, groups, networks, websites, stakeholders that families can be signposted too.
• To use social prescribing to help improve patient outcomes in a person-centred way, accessing the right help at the right time.
• To actively participate in meetings, contributing to the Dynamic Support Register to update all agencies on progress as well as identifying obstacles and barriers to accessing appropriate provision
