Job summary

Lanfranc Medical Centre is looking for an experienced, motivated and enthusiastic Practice Nurse to join our friendly and supportive primary care team on a part-time basis for 10 hours per week.

The successful candidate will play an important role in delivering comprehensive, high-quality nursing care to our patient population and will work collaboratively with GPs, Healthcare Assistants and the wider practice team to provide safe, effective and patient-centred clinical services.

We are looking for an experienced Practice Nurse with strong clinical skills who is able to work both independently and as part of a multidisciplinary team.

Main duties of the job

The successful candidate will be responsible for providing a range of general practice nursing services, including:

• Chronic disease management, including reviews for diabetes, asthma, COPD and hypertension
• Cervical screening/cytology
• Childhood and adult immunisations and vaccinations
• Administration of medications, vaccinations and injections in accordance with relevant protocols and guidelines
• Wound care and dressings
• Venepuncture and other routine clinical procedures
• NHS Health Checks
• Health promotion, lifestyle advice and patient education
• Maintaining accurate, clear and contemporaneous clinical records
• Supporting infection prevention and control, clinical governance and quality improvement
• Working collaboratively with GPs, Healthcare Assistants and the wider multidisciplinary team
• Undertaking other appropriate Practice Nurse duties in accordance with competency, training and the needs of the practice

Job Responsibilities

The post holder will be expected to:

Clinical Care

• Provide high-quality, evidence-based nursing care within their professional competence.
• Undertake chronic disease reviews and support patients in managing long-term health conditions.
• Undertake cervical screening in accordance with current national guidance and practice procedures.
• Administer childhood and adult immunisations and vaccinations in accordance with current guidelines and authorised protocols.
• Undertake wound assessment, wound care and dressings.
• Undertake venepuncture and other appropriate clinical procedures.
• Carry out NHS Health Checks and provide appropriate health promotion and lifestyle advice.
• Administer medications and injections safely and appropriately within the post holder's professional competence.
• Recognise situations requiring referral to, or advice from, a GP or other appropriate healthcare professional.

Patient Care

• Provide compassionate, respectful and patient-centered care at all times.
• Support patients in improving and maintaining their health and wellbeing.
• Provide appropriate health education and advice to patients and carers.
• Maintain patient dignity, privacy and confidentiality.

Clinical Governance

• Maintain accurate, contemporaneous and confidential patient records.
• Work in accordance with practice policies and procedures and relevant professional requirements.
• Follow infection prevention and control procedures.
• Participate in clinical governance, audits and quality improvement activities where appropriate.
• Maintain professional competence and undertake appropriate continuing professional development.

Team Working

• Work effectively as part of the multidisciplinary practice team.
• Communicate effectively with GPs, nursing colleagues, Healthcare Assistants, administrative staff and other healthcare professionals.
• Contribute positively towards a supportive and professional working environment.
• Attend appropriate clinical or practice meetings as required.
