Job summary

The Lead Clinical Practitioner (LCP) is a senior leadership role within Severnside Integrated Urgent Care (IUC), providing advanced clinical practice across The Burren, Out of Hours, Clinical Assessment Service (CAS), Clinical Medication Delivery Unit (CMDU) and ACE services. The role combines clinical duties with leadership responsibilities, including supervision, mentoring, clinical governance, audit, service improvement and patient safety.

Within The Burren, the post holder will work as an autonomous prescribing clinician, delivering primary medical care to patients discharged from secondary care who require rehabilitation and ongoing clinical support. The role requires advanced assessment, clinical decision-making, risk management and chronic disease management skills, supported by the Nursing and AHP leadership team and primary care GPs.

The post holder will act as a key member of the multidisciplinary team, participating in ward rounds and managing routine and urgent patient needs. Working closely with nurses, AHPs, GPs, therapists, social care and operational teams, the LCP will support integrated care, improve patient outcomes and ensure patients receive the right care at the right time. The post includes 16.75 clinical hours and 6 leadership hours per week.

Other responsibilitiesThe primary focus of this role is to provide clinical services to patients, however there are a wealth of opportunities to engage with and drive initiatives, take leadership roles and contribute to service improvement.

Supervision and SupportLead Clinical Practitioners working within SevernSide services will participate in service leading feedback from the Clinical Guardian solution. This provides regular peer to peer feedback on a random sample of notes for all BrisDoc clinicians which are reviewed against set criteria.All Lead Clinical Practitioners will be expected to participate regular clinical forums and supervision both during shift and as a CPD offerThe post holder will have access to Connecting Care, EMIS, CLEO, Nourish, NHS Remedy, AccuRx (Picture and Video Consultation software), MiDoS, Toxbase, PACCS and the BrisDoc Clinical Toolkit to support clinical practice.

Leadership and ManagementLead Clinical Practitioners will manage a team of Clinical Practitioners, providing regular one to ones, clinical supervision, feedback and support as part of the Nursing & AHP Leadership TeamLead Clinical Practitioners will be responsible for areas Quality Improvement to ensure service improvements and be part of driving change within the organisationLead, participate and provide representation in senior Lead meetings to ensure and drive best practice, using the best available evidence to provide exemplary world class care to every patient
