Job summary

We are seeking an experienced and autonomous Urgent Care Practitioner to join our team. This role is ideal for an experienced clinician from an ambulance, urgent care, emergency, community or primary care background who is confident in assessing and managing undifferentiated and acute presentations within their professional scope.

A key aspect of the role is providing same-day care through home visits, practice-based assessments and remote consultations. You will independently assess patients, formulate differential diagnoses, manage clinical risk, initiate treatment, request investigations and make appropriate referral or escalation decisions. An Independent Prescribing qualification is desirable.

You will work closely with GPs and the wider multidisciplinary team to support safe, effective care and help reduce unnecessary hospital admissions wherever appropriate.

Applicants must be a registered healthcare professional (HCPC, NMC or equivalent) with substantial post-registration experience and a proven ability to work autonomously in assessing and managing acute and urgent presentations. A full UK driving licence and willingness to undertake home visits are essential.

Duties and Responsibilities

Provide autonomous assessment, diagnosis and management of patients presenting with urgent, acute and undifferentiated health needs. Develop treatment plans, initiate appropriate interventions, manage clinical risk and ensure safe referral or escalation when required.

Home Visiting and Community-Based Care

Undertake home visits for patients unable to attend practice, delivering comprehensive clinical assessment and treatment in community settings. Support care closer to home and help prevent unnecessary hospital admissions where clinically appropriate.

Same-Day Access, Triage and Clinical Prioritisation

Provide timely same-day care through face-to-face, telephone and remote consultations. Assess urgency, prioritise patients according to clinical need and contribute to effective patient navigation across services.

Communication and Multidisciplinary Working

Work collaboratively with GPs, nurses, pharmacists, community teams and other healthcare professionals to deliver coordinated, patient-centred care. Communicate clearly with patients, carers and colleagues to support informed decision-making.

Delivering a Quality Service and Clinical Governance

Maintain high standards of clinical practice through accurate record keeping, evidence-based care, patient safety activities, audit and quality improvement initiatives. Contribute to the ongoing development of urgent care pathways and services.

Professional Practice and Development

Maintain professional registration and continuing professional development requirements. Participate in clinical supervision, reflective practice and learning activities to support the delivery of high-quality urgent care services.

Information Governance

Ensure accurate use of clinical systems and maintain confidentiality in line with information governance and data protection requirements, supporting safe and effective patient care.

Health, Safety, Infection Prevention and Equality

Work in accordance with organisational policies relating to health and safety, lone working, infection prevention and safeguarding. Promote equality, diversity and inclusion, ensuring equitable access to care for all patients.
