Job summary

You willbe a core member of the multi-disciplinary/multi-agency Hope Service, participating in the response and provision of crisis/urgent assessments, care planning, risk assessing and developing a risk management plan and treatment plan of Children and Young People (CYP) requiring support for emotional, mental health and social care needs. Often this response will be to facilitate a diversion from Psychiatric Hospital admission or an out of area placement and allow for recovery as the CYP remains in their home/placement/community/school. There is also a significant element of Care Coordination for CYP known to the service for an indefinite period of time.

This position would suit an individual who is passionate about delivering high-quality, evidence-based interventions and achieving positive outcomes for CYP experiencing moderate to severe mental health difficulties.

Applicants with a suitable level of knowledge, skills, and experience may be appointed directly at Band 7. Alternatively, the post is available as a Band 6 developmental role, with a structured development pathway to Band 7 within 12 to 24 months, subject to the achievement of agreed competencies and successful performance against the developmental framework.

Main duties of the job

• Care Coordination of young people aged 11-18 in the community who are experiencing Mental Health Crisis
• Developing and strengthening care networks for young people known to the service
• Preventing admission and facilitating discharge from Psychiatric wards for young people
• Contributing to the clinical governance of the service and the Children and Young People's Division in the the Trust
• Supporting families to have the resources to support their children with Mental Health concerns
• Contribute to the evaluation of the service
• Recognise and support safeguarding processes
• Provide clinical leadership to Day Program component of service and junior clinical staff

Job description

Band 6 Key Responsibilities

• The post holder will work closely within the Mindworks Alliance during office hours, responding to crisis and urgent referrals of CYP or for crisis and urgent presentations of CYP already working with the Hope and Extended Hope Services.
• The post holder will work as an autonomous practitioner, undertaking clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided to meet their current needs.
• Work closely with the Emergency Departments, Crisis Intervention Service, Extended Hope, CCISS, the Adult Psychiatric Liaison Service, other Mental Health Crisis/urgent Teams, On Call Duty Psychiatrist, On Call Managers, Paediatric Liaison Nurses, ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed.
• To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of CYP with serious and complex mental health difficulties in crisis.
• To manage own caseload from referral to discharge/transfer, holding medium to high-risk cases and ensure timely and accurate documentation is completed.
• Ensure that outcome measures for the Hope Service are implemented and audited. Contribute to relevant audit/research and ensure that interventions on the pathway are evidence based.
• Interface with other services/agencies regarding care, treatment and discharge planning.
• Support integrative therapeutic goals established within care planning.

Band 7 Key Responsibilities (in addition to Band 6)

The post holder will work closely with the Mindworks Alliance during office hours, responding to consultations or referrals received for potential Hope Service involvement:

• Report to and advise referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.
• Represent the Hope Service at professional/network meetings to offer advice/consultation on complex cases who are in crisis.
• Collaborate on developing systems to monitor and evaluate the service.
• Supervise other junior members of the team and contribute to their appraisal process.
• To undertake relevant CPD to improve assessment and treatment skills.
• Be competent in conducting assessments in all settings.
• Deputise for the Clinical Lead Nurse when required.
• Take responsibility for some core practice development portfolios and essential health & safety domains.

Other duties:

• To actively formulate a robust crisis and contingency plan aka My Safety Plan with CYP and their families/carers inclusive of risk assessment and management and positive risk taking.
• Collaborate on the development and lead on the implementation of clinical guidelines, protocols relevant to sphere of responsibility.
